Band 7 Senior Physiotherapist/Occupational Therapist We are looking for an enthusiastic professional who can help the Community Independence Service (CIS) make Hammersmith & Fulham (H&F) a great place to grow old. The Community Independence Service is co-located with London Borough of Hammersmith and Fulham (LBHF) Adult Social Care on King St, Hammersmith as well as within Charing Cross Hospital. Our service works closely with our acute partners, mainly Charing Cross but also other acute Imperial College Healthcare NHS Trust sites and Chelsea & Westminster Hospital NHS Foundation Trust. You will be connected with peers across the tri-borough of Hammersmith & Fulham, Westminster and Kensington and Chelsea to support you and to work on service improvement partnerships. As the senior physiotherapist/Occupational therapist for the CIS Home first team, you will be expected to be a clinician with a broad range of clinical and line management competencies who is able to adapt to a wide variety of circumstances. You will use evidence based principles, current best practice and specialist skills to assess, plan, implement and evaluate interventions and to optimise patient safety following their discharge from hospital. You will deliver skilled physiotherapy or occupational therapy care to clients in their own home, based on a thorough holistic and risk assessment of their clinical and personal needs and the agreement of an individualised care plan. Main duties of the job To prioritise factors involved in the clients presenting condition through clinical reasoning based on knowledge of current professional opinion, client's prognosis and other physical, social, emotional and mental health issues. To manage the effective coordination and continuity of client care working closely with external agencies and community colleagues. To ensure that clinical practice is contemporary to meeting the needs of clients and families. To ensure that each client has a comprehensive assessment of need, including risk assessment and that their care is culturally sensitive and addresses issues of diversity. To work closely with the acute trust, GP, nursing and therapy colleagues to prevent inappropriate hospital admissions. Attend regular MDT and daily handover meetings as appropriate. To use knowledge of other multi-disciplinary roles and external agencies to ensure effective referral management and achieve best client care. To evaluate your practice through audit and outcome measures with support of senior colleagues when required. To maintain and develop specific departmental duties as designated by pathway lead. Person Specification Education and Qualifications Degree / Diploma Physiotherapist or Occupational Therapist Registration with HCPC as a Physiotherapist or Occupational Therapist Evidence of relevant continuous professional development Member of CSP or RCOT Leadership Qualification Knowledge and Experience Extensive post qualification experience of client care in an acute or community setting Evidence of working in a multidisciplinary team. Knowledge of current national guidelines on hospital discharge process. Extensive knowledge and understanding of common conditions affecting our frail and elderly patients Extensive knowledge and understanding of multi-pathologies and co-morbidities that affect people with long term conditions. Knowledge and understanding of clinical governance procedures, risk assessment and local policies. Experience of clinical triage. Previous community experience Experience of a discharge to assess model of care Skills and Abilities Excellent communication skills within the multi-disciplinary team, ability to present cases, take note of actions, update others in the team and use handover effectively. Excellent communication skills across services and understanding of differing roles and responsibilities and ability to understand other points of view and adapt your own accordingly. Ability to maintain accurate, timely, concise and legible clinical records both electronic and handwritten which adhere to the Trust policie Ability to talk to people about sensitive issues in a supportive manner but also challenge others assertively where necessary. Knowledge and ability to develop and record care plans that are appropriate to the people concerned, identify the risks that need to be managed and have clear goals Knowledge and ability to use Microsoft Office suite for various administration tasks, including spreadsheets, presentations, document writing and email. Knowledge and ability to use client record systems such as Systm1 or Rio. Understanding and ability to apply relevant knowledge and skills required to act as an effective professional clinician and adhere to your relevant code of conduct/practice. Ability to plan and use assessment methods that are appropriate for complex needs, and uses processes of reasoning that are based on available evidence including national guidelines such as NICE and other local and professional policies Ability to follow processes of reasoning which balance additional information against the overall picture of the individual's needs over a fast paced and changing environment. Knowledge and understanding of the role of rapid response services in admission avoidance and early supported discharge. Ability to work well with other members of the team including communicating effectively, being kind and considerate, reliable and enthusiastic Demonstrates flexibility, problem solving, initiative and leadership skills within a fast paced responsive service. Ability to undertake management tasks and leadership of the team, including deputising for the Pathway Lead when required.
