Birches Green, United Kingdom Birmingham and Solihull Mental Health NHS Foundation Trust Full time

Job Overview An exciting opportunity has arisen for a highly motivated individual to work part of the delivery of a new discharge pathway pilot. You will work with a team of professionals to develop a community forensic step-down accommodation for women being discharged from secure care. The Pilot aims to extend the inpatient treatment model (the SCALE model within the Women's Secure Blended Service (WSBS)) into a community residential setting, to promote recovery beyond discharge and reduce the need for future admission. The project has funding for a 2-year period. Responsibilities Co‑facilitate planned therapy programmes focusing on developing independent living skills and supporting integration into the community, forming relationships, and having purposeful occupation within the SCALE model. Ensure effective communication pathways exist, acting as a role model for others to positively reflect the values of the organisation. Work across Ardenleigh and Dawn House to strengthen relationships and ensure effective discharge pathways. Collaborate with external partners in planning interventions, assessments, and joint working with ANAWIM staff and the FIRST and inpatient OT team. Maintain accurate clinical records and appropriate data. Promote OT services and support visitors and service users to understand their treatment. Contribute to assessments through observation, communication, and accurate record keeping. About the Trust Welcome to Birmingham and Solihull Mental Health NHS Foundation Trust. Our 4000 clinical and support staff help us to improve mental health wellbeing and meet the needs of the 70,000 people we serve each year. We provide a range of mental healthcare services across Birmingham and Solihull, as well as specialised services nationally. #J-18808-Ljbffr
