You will bring to the role: knowledge and understanding of current learning disability practice and the role of a multidisciplinary team, including statutory and voluntary agencies skills in effective assessment, planning, intervention and evaluation of work supporting people with learning disabilities ability to build good working relationships with professional colleagues, donors, carers and the wider community experience of using a person-centred, holistic approach and creativity in problem‑solving and reflective practice techniques About the role As a Nurse Care Manager/Social Worker you will be central to a network of partners working across health and social care in Leeds who are committed to integrating services to support people with learning disabilities. On the basis of assessed needs and risks, devise and implement service and support plans which correspond to the needs of service users, their families and carers. Analyse information to support the ongoing needs of service users, families and carers. Monitor and contribute to the review of plans relating to the assessed needs of service users, families and carers. Work within the Safeguarding Framework to investigate and manage risks where individuals have experienced or may experience significant harm. Prepare and provide concise, informative reports based on an analysis of complex evidence that can be used within the legal context. Maintain and update case notes and other records using management information systems to keep records up to date. Provide concise and accurate information about circumstances and plans within data‑protection limits. Apply all relevant practice standards, policies and procedures and maintain compliance with governance. Develop and maintain effective working relationships with internal and external professionals, ensuring an integrated, holistic and multidisciplinary approach to care management and safeguarding. Contribute to promotion of improved outcomes for individuals, families, carers and groups from diverse and disadvantaged communities. Plan and negotiate a range of appropriate services and interventions that meet the needs of service users within delegated levels of responsibility. Identify and implement strategies for responding to concerns about practice or procedures, seeking guidance if required. Participate in meetings and relevant forums. Organise work activities, prioritising tasks and responsibilities to meet statutory legislation and service priorities. Work within the prescribed Professional Capabilities Framework and maintain performance through supervision and appraisal. Undertake assessments including safeguarding investigations deemed suitable for allocation to a newly qualified worker, wherever possible, considering Self‑Directed Support options. Assist in the development of protection plans and commission services designed to minimise risks and improve the quality of life for service users and their carers. Contribute to development of practice and services through participation in workshops and consultations. Qualifications Appropriate professional qualification (Registered LD nurse or dual LD Nurse / Social worker qualification) and registered with the Nursing and Midwifery Council (NMC) or Social Work England at commencement and maintain registration. Essential Requirements Demonstrated ability to understand how to prepare a young person for adulthood, utilising a creative approach to maximize their independence skills. Ability to form relationships with young people and their families, manage change and work alongside other professionals. Effective planning, managing, prioritising and evaluating work with users. Excellent communication and the ability to produce records and reports to a high standard. Strong organisational and time‑management skills, prioritising own and team work to meet conflicting deadlines. Engagement of individuals, families, carers and workers from other agencies. Identification of appropriate social work interventions to promote positive change. Development and implementation of service and support plans. Assessment of needs using a person‑centred approach. Accurate record keeping and adherence to policy and procedure. Use of technology for evidence and communication. Judgement, risk assessment and appropriate consultation. Use of supervision to reflect critically on practice and explore alternative approaches. Knowledge of theories and models for social work intervention. Knowledge of relevant legislation, guidance and policy, with consistent application. Awareness of factors impacting service users and families. Understanding of statutory and voluntary agency roles. Basic IT proficiency (Microsoft Office, Internet, Email). Knowledge of current trends, policies, legislation and regulatory bodies for vulnerable people. Understanding of confidentiality, safeguarding and diversity issues. Creativity in problem‑solving, advocacy, user involvement and person‑centred practice. Initiative and commitment to developing services. Reflective practice techniques to inform decision‑making and planning. Experience of information gathering for assessments. Effective use of supervision. Minimum of 12 months post‑qualification experience in a statutory/health setting.
