OverviewAn exciting opportunity has arisen to join the Mid Specialist Dementia and Frailty pathway team within Mid Essex. We are looking to recruit a motivated, hardworking team leader who has excellent communication skills with a passion for driving service improvement and working collaboratively with other health care providers and third sector agencies.The successful candidate will be responsible for leading the Chelmsford team, ensuring the delivery of a high level service providing a proactive assessment, diagnostic, monitoring and management of patients with dementia of any age and people over the age of 75 years with complex Mental Health needs who have an age related frailty.ResponsibilitiesProvide leadership of the Multi Disciplinary Team to deliver the serviceResponsible for the overall delivery of a range of evidence based interventions to service users based on identified holistic needs, review the effectiveness of implemented care packages and discharge service users back to primary care / other providers in a timely manner.Develop robust interagency relationships with key stakeholders supporting development of the integrated care pathways and locality working.Responsible for the coordination and management of the team in providing a safe, effective and therapeutic clinical service, ensuring the delivery of high quality community mental health services.Responsible for general service management including: appraisals, supervisions, complaints/investigation management, management of sickness absence/disciplinary/capability and other workforce procedures and policies.Support others in their learning and development in relation to the management needs of patients with dementia and serious mental illness with age related frailty.To ensure the service works in line with CPA/Social care/risk management/safeguarding and other relevant policies.QualificationsAn experienced registered mental health nurseA competent, confident and proficient practitionerWho is organized and able to work creatively and effectively in a busy and demanding roleIt is essential that you have a UK driving license and able to drive for this position.BenefitsSeason Ticket LoansNHS discounts for staffExcellent Training facilities and opportunitiesBuying and Selling annual leave schemeThe opportunity to work bank shifts and expand knowledge and experience in other areasSalary Sacrifice schemes including lease cars and Cycle to WorkDay One Flexible EmployerFlexible working and staff bankThe Trust supports and actively encourages flexible working for all employees.
