Community Heart Failure Specialist NurseWe are seeking an experienced and motivated Community Heart Failure Specialist Nurse to join our expanding service. This is a rewarding opportunity to make a significant difference to patients living with a confirmed diagnosis of Heart Failure--supporting them to manage their condition safely at home, reducing unplanned hospital admissions, and improving long-term quality of life.The post holder will provide expert clinical knowledge and specialist nursing skills in the management of Heart Failure across the community setting. You will hold a defined caseload, delivering comprehensive assessment, treatment optimisation, education, and follow-up to both newly diagnosed and existing patients.Working autonomously--yet fully supported by the wider MDT--you will:Provide proactive management of patients to avoid unplanned hospital admission.Optimise medication and treatment plans in line with national and local Heart Failure guidelines.Deliver patient and carer education to support confidence and self-management.Liaise closely with GPs, secondary care cardiology teams, community nursing, therapy services and social care to ensure seamless, coordinated care.Advocate for high-quality Heart Failure care in line with NSF standards and contribute to ongoing service development and strategic planning.Main duties of the jobUse advanced clinical skills and evidence-based knowledge to identify investigation and/or treatment needs of individuals.Provide medicines management support and advice in accordance with national and local guidelines.Provide appropriate advice and support to patients and their carers including the provision of education and the promotion of self-management skills.To liaise with local cardiac rehabilitation teams and End of Life teams as appropriate.Act as a specialist resource for patients, their carers and other health professionals to improve the management of patients with heart failure.Liaise closely with primary and secondary care services to provide an integrated care plan in which the patient and carers participate.Ensure mechanisms in place for escalation of care at times of clinical deteriorationTo triage referrals from GPs, consultants, inpatient heart failure team and other professionals and service in agreement with individual patient.Provide empowerment and support, acting as an advocate to enable individuals and their carers to actively engage in agreed treatment programmes.About usFrimley Health NHS Foundation Trust is committed to being an inclusive and disability confident employer and has been awarded the Gold for the Armed Forces Employment Recognition Scheme.
