In this pivotal role, you will ensure that care and support services for adults meet individual needs, promote independence, and deliver value for money—all while maintaining compliance with statutory requirements.Key ResponsibilitiesReview Care Plans: Conduct scheduled reviews of care and support plans to ensure outcomes are met and services remain appropriate.Engage with Service Users: Communicate with service users, carers, and providers to gather feedback and assess satisfaction.Monitor Service Delivery: Ensure commissioned services are delivered as agreed and meet quality standards.Identify Changes in Needs: Highlight cases where needs have changed significantly for reassessment by qualified social workers.Record Keeping: Maintain accurate and timely records on the Council's case management system.Compliance: Ensure all activities comply with the Care Act 2014, safeguarding principles, and Hackney Council procedures.Collaboration: Work closely with social workers, providers, and commissioning officers to drive continuous improvement in service delivery.Reporting: Provide data and insights to support commissioning decisions and performance monitoring.What We're Looking ForStrong organisational and communication skillsAttention to detail and the ability to manage multiple casesCommitment to improving outcomes for adults receiving care servicesKnowledge of the Care Act 2014 and adult safeguarding principles is desirablePlease do forward your most recent CV to (url removed) or call now on (phone number removed). #J-18808-Ljbffr
