Vine Medical Group is recruiting for a full or part time Advanced Nurse Practitioner to join our Same Day Care Team.Working hours can be between 30-37.5 hours per week with a working pattern to be agreed that would include Mondays and Fridays.Our new team member will work both independently and withina multi-disciplinary team environment, to provide safe, holistic care to ourpatients. This will include remote and face to face appointments, reactive andproactive home care visits. To include history taking, physicalassessment and treating minor illness and ailments, undertaking investigationsto inform clinical decision-making to establish a diagnosis and management planin collaboration with the patient and their families or carers.To request diagnostic investigations and interpret within agreed protocols e.g.x-ray, blood tests, scans, etcWork within clinical boundaries, demonstrating safe and competent clinicaldecision-making, delivering high quality clinical care and maintaining patientconfidentiality at all timesProvide telephone or video consultations, including the safe and effectivetriage of acutely unwell patientsConduct reactive home visits to those acutely unwell patients, assess, plan,treat and arrange admission to hospital if required, for the practice andwithin the PCNFuture intention to conduct proactive home visits to housebound patients withlong term conditions, including but not exclusively dementia, diabetes,respiratory disease.Make appropriate referrals to primary, secondary and social care services,following practice (and/or locally agreed) pathways and protocols.Whereappropriately qualified:Supply,administer and prescribe medicines as indicated (and within competencies) toaddress patient need, in accordance with medicines management policy, patientgroup or patient specific directives or NMC standardsPromote self-care and educate service users to allow them to be empowered tomake informed choices about treatmentTo appropriately request and collect pathological & microbiology specimensincluding venepuncture, swabs, urine culture in line with local guidelines andprocess results as required actioning them in a timely manner in line withprotocolsIdentify, manage, and support patients at risk of developing long-termconditions, preventing adverse effects on the patients health, promotepatients to live healthily and encourage principles of self-careRecognise and manage anaphylaxis, according to the current UK ResuscitationCouncil guidelinesPerform Cardio-pulmonary Resuscitation and the correct use of Automated Defibrillatorfollowing training, according to the current UK Resuscitation CouncilguidelinesTo provide general health promotion education, advice on national screeningprogrammes and guide/refer patients to appropriate services or other healthprofessionals as deemed appropriateProvide information and advice on prescribed or over-the-counter medication, onmedication regimens, side effects and interactions as appropriate and withinyour level of competencySupport times in accordance with legislative requirements andPractice Policy and ProceduresPerson SpecificationQualificationsRegistered Nurse Practitioner / Advanced Nurse Practitioner with NMCQualified to Master's degree or equivalentFull UK Driving LicencePhysical Assessment and Minor Illness TrainingAccredited training in any of the following: Urgent care, Chronic disease management or frailtyNon-Medical PrescriberKnowledge & SkillsClinical examination skills across a diverse range of patients including both children and adultsAble to work autonomously within your scope of practice whilst also having an understanding of when to access the wider clinical teamAble to build positive rapport with a diverse range of patients and colleaguesGood organisational and time management
