Assistant Practitioner - Falls Prevention and Management Therapy Closing date is 10 December 2025 This is a fantastic opportunity for an assistant practitioner to join the team as we expand our service. You will be a part of the falls prevention service across CTM, providing falls advice, assessment, and intervention at local venues. The service focuses on both prevention and intervention, building integrated teams and designing a seamless pathway for our patients. You will be valued as an integral part of the service development and supported in developing your skills. Main duties of the job Support Allied Health Professions (AHP) – Physiotherapists and Occupational Therapists – in delivering AHP intervention for patients in the community to prevent and manage falls. Work as part of the CTM Falls Prevention and Management Team to undertake clinical and administrative tasks, liaise with health colleagues, Local Authorities and third sector organisations to ensure care for patients within CTMUHB/ Bridgend/RCT/Merthyr County Boroughs as instructed and delegated by the AHP, responding flexibly to the needs of the service. Assist AHP staff with therapeutic modalities aimed at maintaining patients in the community and preventing avoidable falls. Qualifications Essential Evidence of good education to include English, Mathematics GCSE or equivalent qualification or skills, knowledge, and experience OCN/Agored accredited modules or relevant NVQ level 3 Knowledge relating to a range of core AHP activities Appropriate level of qualifications for the post Knowledge of a broad range of health conditions impacting on function related to falls Desirable Successful completion of relevant competency‑based training Experience Essential Administrative skills Experience of developing/delivering education advice to individuals/groups Experience of multi‑disciplinary team working Experience of working as a HCSW in a medical, rehabilitation or community setting Desirable Experience of falls prevention work Previous experience of NHS, acute clinical or community setting Skills Essential Excellent verbal and written communication skills Numerical skills Excellent ICT, computer literacy, and keyboard skills The ability to work independently whilst under appropriate supervision Desirable Ability to work with patients with communication barriers/motivational issues Welsh language speaking/listening skills at Level 3 or above Personal Qualities Essential Ability to work independently and as part of a team Ability to motivate self and others Confident in approaching tasks Disclosure and Barring Service Check This post is subject to the Rehabilitation of Offenders Act (Exceptions Order) 1975 and as such it will be necessary for a submission for Disclosure to be made to the Disclosure and Barring Service (formerly known as CRB) to check for any previous criminal convictions.
