Release and Transfer Coordinator

About the Role The Release and Transfer Coordinator will support the wider healthcare team at HMP Thameside, a category B male prison in London, preparing patients for release or transferring them to another prison. The role operates Monday to Friday, 08:00 to 16:00, with an annual salary of up to £36,000 per annum FTE. Responsibilities Reduce unnecessary and confusing handoffs between healthcare services and ensure consistency in service transfer between healthcare providers by initiating early discharge planning or transfer to alternative settings, including transitioning existing care plans. Ensure safe transfers between establishments that provide continuity of care and ongoing support. Take patients’ goals and needs into consideration when planning care prior to transfer, and put appropriate actions in place for patients with complex health care needs. Remove delays for patients accessing and receiving the healthcare and medication they need during the transfer process. Qualifications Experience working with patients within the Criminal Justice System. Evidence of implementation of local and national policies, such as NICE guidance, developing protocols and care pathways, and monitoring effectiveness. Knowledge of statutory requirements for safeguarding adults and children. Willingness to innovate, embrace learning through continued professional development, and apply new skills in the workplace.
