Job Summary The post holder will be employed by Greenwich Health the GP Federation in Greenwich. Greenwich has 6 primary care networks (PCNs) with list sizes between 30-80,000 patients. The post holder will work for Riverview PCN, a grouping of 5 practices, and will work across the practices within this PCN. The post holder will work as part of a multi‑disciplinary team in a patient‑facing role. The post holder will work under the supervision of the Clinical Pharmacist and as an integrated part of the primary care team to ensure the safe, accurate and timely supply of prescribed medication to patients. The role will contribute to successful clinical management of patients and build capacity and capability in the workforce at the right levels. Main Duties Adhere to organisation policies and procedures and relevant legislation including the requirements of any professional bodies. Maintain satisfactory personal performances and professional standards and achieve agreed objectives for the role. Attend mandatory training as identified by the organisation. Participate in a Performance Appraisal Scheme and contribute to own and staff development. Maintain professional indemnity suitable for the range of work undertaken. Carry authorisation for making changes to prescribing records within the practice (written agreement or GP signature for audits). Key Responsibilities Provide technical and administrative support to Clinical Pharmacists and Clinicians. Support primary care teams with medication queries from patients, primary and community staff. Work independently under the direction of the clinical pharmacist and manage own workload. Use excellent communication skills to liaise with patients, primary care staff, community pharmacists and other partners. Work with the PCN Clinical Pharmacist in accordance with agreed prescribing SOPs. Support specific work on prescribing initiatives and promote cost‑effective, safe, evidence‑based prescribing. Participate in monitoring progress against key performance targets. Contribute to the development of the role. Direct Patient Services Process repeat medication requests, convert acute requests to repeat medicines, change quantities, sync patient medications, e‑RD, arrange compliance aids. Issue prescriptions in line with agreed clinical protocols and direct queries to appropriate clinicians. Provide medication review services via clinics, domiciliary visits, residential and nursing homes. Maintain accurate patient medication records on the practice clinical computer system. Provide advice on medication optimisation and lifestyle (e.g., inhaler techniques). Support safe and effective repeat prescribing of high‑risk medicines. Conduct medication reconciliation after discharge or care transfers. Use sensitive communication to ensure patients consent to treatment. Deliver high‑standard, patient‑centred care. Qualifications Registered with GPhC. BTEC/NVQ Level 3 or equivalent in pharmacy services. Alternative approved training pathway (e.g., PCPEP or MOCH). Experience Evidence of ongoing CPD and commitment. Significant post‑qualification experience as a technician in primary care, hospital or community pharmacy. Ability to dispense prescriptions to high standard in a pharmacy dispensary setting. Excellent verbal and written communication skills. Negotiating and interpersonal skills. Competency in IT and common software (e.g., MS Office). Ability to work independently, prioritise, and meet deadlines. Ability to travel across the PCN. Knowledge and Skills Understanding of GP practice operations and clinical governance. Accurate clinical note taking. Strong IT and Office/Outlook skills. Excellent communication, telephone manner, problem‑solving, and analytical skills. Knowledge of medicines optimisation principles and pharmacy law. Understanding of national policies impacting medicines management.
