Job Summary An exciting opportunity has arisen for a proactive and dynamic candidate to provide operational leadership and line management to the Community Access Team, which is based at the Norfolk and Norwich University Hospital NHS Foundation Trust. The Team Lead will manage the workforce, take responsibility for quality monitoring and work collaboratively with system partners. Our team consists of Senior Nurses, Discharge Co‑Ordinators, Senior Physiotherapists, Senior Occupational Therapists and Assistant Practitioners. At the front door, we provide an Admission Avoidance service from 8AM to 8PM, 7 days a week. At the back door, we support patient transfers from Norfolk and Waveney acute hospitals to our Intermediate Care beds across Norfolk. The successful candidate must be able to make own travel arrangements. Please note that there isn't parking available on site at Norfolk and Norwich Hospital. Responsibilities Day‑to‑day operational line management of the team. Planning, coordinating and evaluating services to ensure they are safe, efficient and effective. Facilitating development of the integrated team and individual staff to promote job satisfaction and performance. Performing absence management and reporting in a timely manner in line with Trust policy. Deputising for the Integrated Service Lead as required. Collaborating with healthcare professionals and statutory/non‑statutory agencies to provide a seamless, integrated service. Adapting communication style to meet the needs of patients, staff and colleagues. Maintaining strong, transparent working relationships with key stakeholders. Monitoring and managing the team budget. Proposing and implementing departmental policy changes for management of patients, networking with specialist practitioners. Representing the service and NCH&C at strategic meetings with partner organisations. Managing the Community Access Team caseload. Qualifications Registered Nurse, Physiotherapist or Occupational Therapist. First level degree, diploma or equivalent, or post‑registration experience such as a DN qualification. Recognised Management Qualification or evidence of working at that level. Current registration to a professional body. Desirable: Masters level degree. Experience Experience of leading a team. Experience of working in a multi‑agency environment. Experience of managing budgets. A track record of successful service and policy development and implementation. Evidence of post‑graduate development. Performance management experience. Desirable: Experience of managing a multi‑disciplinary team. Desirable: Project management. Physical capacity to kneel, bend, stoop and work in cramped environments. Ability to manoeuvre limbs of around 5‑6kg and handle patients using aids. Basic IT and standard keyboard skills.
