ThePractice is currently experiencing rapid growth and is undergoing a transitionperiod having recently merged with another practice. Manor Practice is a friendly, well organised and thriving Practicethat benefits from a growing patient list size. The GP partners are dedicatedto providing the highest standard of care to their patients and are supportedby an excellent team of salaried GPs, Practice Nurses, HCAs, Clinical Pharmacists, receptionists and administrators. We also work closely with a number of otherPCN ARRS roles such as FCP Physios, Health & Wellbeing Coaches, SocialPrescribing Link Workers and Mental Health Practitioners. The Practice is an active member of the Wallington PrimaryCare Network and is rated as Good by the Care Quality Commission. We are a forward thinking practice, whoappreciates the need for work/life balance, clinical supervision and supportand we also support flexible scheduling options. Our local Training Hub ensuresour staff have access to excellent training and development opportunities andportfolio working arrangements. We are fully engaged with our local Primary Care Networks,including provision of localised services and we host some of the Suttonborough extended access services at one of our sites. Job responsibilities The ANP will work autonomously within theirprofessional scope of practice providing care for patients face to face, bytelephone consultation and via home visits. The postholder will operate within their own sphere ofcompetence and agreed acceptable limits of practice to provide expertprofessional clinical advice to patients, carers and colleagues and ensuringclinical safety and excellence at all times. The ANP will prioritise and triage the needs ofpatients and make any necessary referrals for advice, treatment orinvestigations in the appropriate manner. The postholder will use skills, knowledge and competenciesas a fully qualified ANP and non-medical prescriber in order to beresponsible and accountable for managing patient caseloads, referrals,admission to hospital if required and discharge from care as needed. In accordance with the practice timetable, the post-holderwill undertake a variety of duties, including: face to face consultations, telephoneconsultations and answering queries visiting patients at home in relation to acuteand chronic presentations participating in triage and reviewing incomingpathology results and clinical letters prescribe and review medication that isappropriate to the patient needs and in accordance with the latest evidencebased practice, national and practice protocols and within the scope ofqualified practice dealing with paperwork and correspondence in atimely fashion Assessing the health care needs of patients withundifferentiated and undiagnosed problems Screening patients for disease risk factors andearly signs of illness Developing care plans for health in consultationwith patients and in line with current practice disease management protocols Providing counselling and health education andsupporting health screening programmes Recording clear and contemporaneous consultationnotes to agreed standards Acting appropriately in relation to alerts, QOF,medicines management, local enhanced services and other reminders during aconsultation Undertaking special health care reviews such asSMI health checks, Learning Disability health checks and cancer care reviews.
