Diabetes Specialist Nurse

Honiton Surgery are looking for an experienced diabetes nurse to join our practice team.We are looking for someone for 19hrs a week, to lead on the management of our diabetes patients. This would suit someone with experience of working with diabetes patients in a hospital, community or primary care setting.Main duties of the jobWorkingwith the nursing team, as a member of the practice multidisciplinary team, thepost holder will ensure practice diabetes services are delivered effectively tothe our patients.Your main responsibilities will include:Delivering nurse led clinics to assess,diagnose, plan, implement and evaluate treatment/interventions for patients forpatients presenting with diabetes, pre- diabetes or undifferentiated diagnosis.In collaboration with other members ofthe practice team, lead the planning and delivery of diabetes services,promoting integrated and seamless pathways of care.Initiate Insulin and GLP1s whereindicatedProvide support and mentorship to othercolleagues within the practiceContribute to practice targets bothlocal and national, e.g. QOF.Refer patients to other service/agencies as appropriate.Promote health and well-being throughthe use of health promotion screening.About usWe are a very busy, well-established GP practice of 17,749 patients and are located in the market town of Honiton in East Devon. We have 5 GP Partners, 13 Salaried GPs, 1 Advanced Nurse Practitioners, 1 Lead Practice Nurse, plus all the associated teams that incorporate our workforce. We have active participation in PCN development with two other local practices and have a good rating with CQC.The Partners and Practice Manager are innovative and forward thinking, and always open to continuing the development of future systems that support excellent patient care and staff wellbeing.We are looking for a committed, enthusiastic individual who is proactive and well organised. This will be dependent uponfactors such as workload and staffing levels:a. To operate nurse led clinics to assess,diagnose, plan, implement and evaluate treatment/interventions for patients forpatients presenting with diabetes, pre- diabetes or undifferentiated diagnosis.b. To maintain accurate clinical records in conjunction with extantlegislationj. To ensure clinical codes are used effectively by all members of thenursing teamk. To ensure the appropriate use of chronic disease registersl. Work to support compliance and adherenceto prescribed treatments by detecting and managing side effects and or adversereactions.m. To request pathology services and process pathology results asrequired.n. To liaise with external services/agencies to ensure the patient issupported appropriately (vulnerable patients etc.).o.
