An exciting opportunity has arisen for a Band 5 nurse to join our innovative South Locality Team. The team offers a range of interventions that are person‑centred and fully collaborative with the patient. The post holder will work as a member of a multidisciplinary Community Mental Health Team, providing services to clients experiencing serious and enduring mental health difficulties. A flexible, patient‑focused approach is essential, with a strong emphasis on building and maintaining professional relationships with colleagues, statutory agencies and voluntary partners. We are looking for an enthusiastic, team‑working individual who is keen to develop their knowledge and skills. The successful candidate will be a competent and motivated practitioner with appropriate, demonstrable experience and a commitment to high‑quality care. PRINCIPAL DUTIES / MAIN TASKS Managing a team caseload under supervision in accordance with sound nursing practice and principles. Undertaking regular supervision and PDR in accordance with Trust policy. Promoting and maintaining the highest standards and attitudes toward the care of patients/clients, and assisting colleagues in maintaining an effective and efficient service. Assisting, developing, implementing and evaluating plans of care, ensuring therapeutic and recreational programmes for patients/clients individually and in groups within the community. Ordering, storing, monitoring and administering medication. Liaising within the multi‑disciplinary care team, including carers, voluntary agencies and other partners. Additional responsibilities include carrying out nursing interventions within the community as required, and supporting the development of the Nursing Team through collaboration with other agencies to ensure the highest standard of patient care.
