**Job Title: Care/Prescriptions Navigator (Apprentice)**
**Normal Place of Work: Frome Medical Practice**
**Accountable to: Care Navigator Team Leader and Pharmacist Technician**
**Job Purpose**:To provide excellent patient support and to navigate patients to the right care and the right time with the right person
The Practice delivers primary care services to nearly 30,000 patients. The Medical Practice is a core part of the local community and is situated in a fantastic purpose built surgery with excellent facilities and a large supportive team. We also have an out-patients department, mental health services, district nurses, pharmacy, and health connectors all collocated with us.
**Main Duties and Responsibilities**:
**Care Navigation**
- Be responsive to the needs of patients and navigate patient care to the correct destination.
- To use both Informatica and EMIS when booking patients
- Ensure call waiting times are kept to a minimum while providing a high quality care navigation service to patients
- To make decisions to promote continuity of care
- To support clinicians where appropriate and check in on them
- Book, rearrange and cancel appointments.
- Work in both the Call Centre and on the Information Desk.
- Use the technology provided to communicate with our patients.
- Deal with requests and tasks from clinical staff (EMIS).
- Signpost to other departments and support networks.
- Deal with incoming and outgoing post.
- Assist with additional clinics e.g. flu, immunisations
- Chaperone appointments when needed.
- Practice checks e.g. security, closures, sharps, fridges.
- Be responsive to and act upon feedback provided during coaching, call listening sessions and developmental feedback.
- To complete evening tasks following switchover to 111.
- To ensure all staff and patient areas are kept tidy and take any necessary action to problems occurring.
- Reasonable ad-hoc jobs on request from accountable persons and Management
- Dealing with visitors
- Attending meetings when required
**Administration Duties**
- To be able to take and pass on messages to a high standard and in a timely manner.
- Auditing and filing patient records.
- Assisting patients with blood pressure monitoring, scales etc.
- Averaging home blood monitoring readings.
- Patient Accessible Information Standard (PAIS ) support for patients’
- Sorting deliveries
- Dealing with samples and specimens
- Contribute as a champion in one or more areas including but not limited to:
Patient Feedback, Health and Well Being, Continuity, Active Signposting and Hello My Name is, Green impact. Contribute to induction and ongoing learning of team in areas of knowledge
**Prescriptions Navigator**
To work as part of the repeat prescription team. To ensure we collectively meet the attainments of prescription targets and ensure high level of patient care. This team is currently being reviewed and the role will become part of a wider medicines management team to help support GP workload and the overall efficiency of the practice.
**Main Duties and Responsibilities**:
- To ensure a high level of service delivery to all our patients.
- Adhere to the Practice repeat prescription protocols
- Process repeat prescriptions according to protocol and electronic prescribing
- Respond to GP led tasks and requests and liaise with GP or Pharmacist responsible for script signing on a daily basis
- Liaise with pharmacists regarding any day to day issues and medication shortages
- Advise colleagues regarding any queries / difficulties experienced by them when producing repeat prescriptions
- Ensure Prescription Team Leader is informed of any back-log of work or if targets are not being met
- Ensure Prescription Team Leader is informed of any situation that hinders the flow of the repeat prescription system.
- Be available to assist in the care navigators as and when required
- Adhere to a strict code of confidentiality in all aspects of the work.
- To help with practice work flow with appropriate coding of letters with medication
- Pharmacist and technicians led tasks
