The Mind Partnership require one more Mental Health Adult Facing Practitioner (AFP) to join their team.

PURPOSE OF THE JOB*
The AFP will engage with parent/carers who require time-limited support with their mental health and wellbeing needs. As part of the multi-disciplinary Family Solution Plus team, you will promote early engagement and aim to de-escalate families in terms of social care intervention.

The AFP will take a holistic and person-centred approach to supporting adults to identify their strengths, set goals and support motivation to improve their mental health and wellbeing through a variety of interventions. By working alongside social care, substance misuse and domestic abuse colleagues, the mental health AFP will be available for consultation and the mutual sharing of good practice between professionals.

RESPONSIBILITIES: *
- Support the delivery of the mental health component within the project, embedding the model as an alternative pathway for service users
- Undertake assessments of parent/carer’s mental health using a suitable, evidence-based assessment tool
- Work in a multi-disciplinary way under the direction of the Social Work Team Manager & Mental Health Team Manager
- Work as part of a multi-agency model with colleagues in the team to assure appropriate, timely intervention
- Deliver interventions specifically related to parental capability, mental ill health and presenting need with individuals and groups
- Undertake home visits and appointments in the community, in line with agreed risk assessment and management plans
- Facilitate and support access to Primary Care and Primary Care Networks
- In line with the Project Manager (FSP), develop and implement crisis plans as appropriate, diverting people away from the use of emergency services
- Facilitate referrals to secondary mental health services and IAPT (Improving Access to Psychological therapies) as appropriate
- Facilitate access to third sector partners and agencies that support with emotional health, debt and employment
- Ensure parents/carers have access to a wide range of information, advice and guidance on general mental health and wellbeing, including developing skills to maintain a balanced approach to wellbeing
- Work collaboratively within the multi-agency framework to maximise the effectiveness of service delivery, attending relevant meetings and providing information and feedback on the mental health component of the project
- Maintain effective professional relationships with the FSP framework staff
- Maintain knowledge of Adverse Childhood Experiences (ACEs) and the identification and possible impact these could have on parenting and mental health
- Record interventions with parents/carers on the OCC Children’s Services IT system (Liquid Logic), ensuring appropriate information is recorded to measure and manage the performance of the service
- Ensure people who access Family Solutions Plus services have the opportunity to engage with, to influence and participate in the running of our services

**Skills and Experience**

**Essential**
a) Excellent knowledge of mental health issues, mental health services and the needs of people living with mental health problems.
b) Knowledge of good practice in safeguarding children and adults and ability to utilise this.
c) Knowledge of the importance of providing performance reports to funders.
d) Ability to be persistent and creative when challenges arise.
e) Ability to be solution-focused and reflective in working practice.
f) Excellent organisational, planning, administrative and IT skills.
g) Excellent communication and interpersonal skills and the ability to maintain positive inter-agency relationships.
h) Effective teamwork skills with the ability to take initiative and work on an individual basis, including adhering to lone working and safeguarding processes.
i) Capacity to cope effectively with the pressures of the post and to use the support and supervision provided.
j) Strong commitment to the involvement of people who use services, social inclusion, recovery, and partnership working.
l) A driving licence and access to a vehicle

**Desirable**
l) Lived experience of mental health (including supporting friends and family).
m) Relevant qualifications or training within mental health and/or children services or service user involvement.
