An exciting opportunity

We are seeking a dynamic individual to work alongside the Discharge to Assess (D2A) Operational Manager supporting discharges via the Single Point of Access (SPoA) from Acute and community hospitals. The Central D2A team is newly formed and is in a transitional phase, bringing opportunity for the right individual to shape and develop processes and ways of working. The team is responsible for ensuring that patients referred to the D2A pathway receive a high-quality assessment of need, ensuring that the patient is at the heart of the assessment, their view and those of their relatives and carers are considered and that the care is provided within national timescales.

The applicant will be a Registered Health Professional and have experience of line managing staff. The applicant must be educated to degree/working towards a degree and experience working in an acute and/ or a community based setting. Furthermore, the applicant must have an understanding of the background to and aims of current healthcare policy, NHS Continuing Healthcare and recent changes to NHS Hospital Discharge Guidance and policy.

This role may entail some travel across the ICS, and a hybrid model of working (from home and office based when required).

To work within the NHS Continuing Healthcare team to establish and maintain robust systems to implement current policy and guidance relating to the NHS Continuing Healthcare processes.
To ensure the preparation of good quality needs portrayals and assessments based on the National Framework Decision Support Tool, to chair panels, manage appeals and present cases to the NHS Continuing Healthcare Panel or NHS England Independent Review Panels.
Monitor numbers of cases awaiting assessment and appeals, generating estimates of potential financial risk and developing reporting mechanisms to ensure Hampshire, Southampton and Isle of Wight Clinical Commissioning Group (HSICCG) is kept updated as part of performance assurance of NHS Continuing Healthcare.
Manage a team of professional staff and administrators and contribute to the training and development of all staff regarding policy and guidance around the NHS Continuing Healthcare and Funded Nursing Care appeals process.
