**Practice Nurse**

We are looking for someone who is enthusiastic and motivated and enjoys working within a team. We have a vibrant multicultural practice population and work closely with the wider multidisciplinary teams.

**Role Requirements**
- They should also be able to demonstrate the ability to cope in a busy and demanding environment.
- The role will include wound care management, childhood immunisations, women’s health, long term conditions management (including COPD, Diabetes, Spirometry and Asthma) and the care of patients who attend general practice.
- Those applicants who are experienced PN’s who may wish to join as a lead PN should be encouraged to apply.

**Role Responsibilities (not limited to)**
- Assisting other clinicians in the management of emergencies and acutely unwell patients to take precedence over booked, routine appointments, e.g. nebulising acute exacerbations of airways disease, carrying out ECGs on patients with acute chest pain, managingacute wounds.
- Acting as chaperone if required by other clinicians; assisting with minor surgical procedures and with IUS/IUD and contraceptive implant procedures including maintenance of equipment and stock for these.
- Cervical screening and opportunistic promotion of breast awareness.
- Administration of adult and paediatric injections (including hormone implants) and immunisations according to Patient Group/Specific Directions (PGD/PSD); ensuring safe storage, rotation, and disposal of vaccines and drugs.
- Wound assessment and dressings, including post-surgical care and removal of clips, staples and sutures; management of complex wounds including onward referral to the Tissue Viability or Dermatology specialist teams.
- Travel health care, including vaccinations and malaria according to PGD/PSDs and other travel advice as appropriate.
- General and specific health screenings for practice patients according to agreed protocols (e.g. new patient registration checks, NHS over 40 health checks) with onward referral where required.
- Provision of lifestyle advice regarding weight loss, activity, smoking cessation, alcohol intake and support for patients motivated to change any of these behaviours with onward referral where appropriate.
- Providing other tasks not specified here if these are with the nurse’s competence and to meet patient need.
