Croydon Council, Adults Social Care, is seeking to recruit a Health and Wellbeing Officer to work in the Adults Early Intervention Support Service (AEISS) leading the way in delivering vital first-point-of-contact services to adults in need of support.
Key Responsibilities:
- Ensure timely and efficient responses to referrals and inquiries received.
- Respond to phone calls and incoming referrals, ensuring that individuals receive the right care and advice promptly.
- Maintain a customer-focused approach, focusing on early intervention, prevention, and supporting independence.
- Ensure that individuals, carers, and families receive clear and accessible information and advice.
- Work closely with individuals and communities to ensure that services are responsive and inclusive, particularly for vulnerable or hard-to-reach groups.
