Job Summary

The post holder will have a lead role in the development and delivery of the Falls service aimed at identifying people at risk from falling and preventing falls and fractures through prevention strategies. They will work as an autonomous practitioner with specialist knowledge and skill in relation to falls prevention and management.

The post holder will be accountable for providing clinical care to patients and advice and support to multi-disciplinary teams, with accountability for ensuring the provision of the highest standards of patient care through ongoing assessment, planning, implementation, and evaluation of programmes of care.

The post holder will be an experienced Allied Health Professional (AHP) or Registered Nurse (RN) who will support the ongoing management in relation to patient falls, identifying and taking a key role in the implementation of medium and long-term strategies to reduce falls, undertaking education and training of staff, audit, and analysis of falls data.

Main Duties of the Job
  1. Provide professional support to clinical teams, delivering evidence-based expert falls prevention advice that supports the provision of the most effective care to patients with a diverse range of clinical conditions, reducing their risk of falling and risk of suffering harm as a result of their fall.
  2. Expected results are defined in the job description but the post holder decides how they are best achieved and is guided by principle and broad occupational policies or regulations. Guidance may be provided by peers or external reference points and clinical supervisors.
  3. Promote activities to prevent deconditioning as well as fall prevention.
  4. Ensure compliance with national guidance - NICE and World guidelines.
  5. Lead the National Audit of Inpatient Falls (NAIF) - identifying patients, reviewing results, and ensuring meeting the KPIs set by the audit.

Job Responsibilities
  1. Contribute to the development of efficient and effective clinical pathways for patients at risk of falls within the Trust.
  2. Lead the Trust Falls Improvement Group and support other meetings as appropriate.
  3. Provide professional support to clinical teams, delivering evidence-based expert falls prevention advice.
  4. Exercise a high degree of professional autonomy and demonstrate the ability to make sound decisions based on knowledge and experience.
  5. Make specific interventions based on evidence appropriate to assessed needs, context, and culture.
  6. Monitor trends and significant issues related to falls and work alongside care groups using QI methodology.
  7. Monitor, review, and audit effectiveness and implementation of falls policies.
  8. Advise managers and employees on current practices and specific falls problems.
  9. Advise ward staff on patient-specific falls issues for care planning and/or complex patients.
  10. Advise on the falls aspects of new projects and equipment purchasing.
  11. Support the coordination of the Trust Falls Improvement Group meetings.
  12. Provide regular reports, thematic analysis, and feedback to relevant managers and the Falls Improvement Group.
  13. Support investigations in which patients have sustained fractures & serious injury.
  14. Maintain clinical and professional skills and standards.
  15. In partnership with patients, make sound decisions ethically based in the interests of patients and clients.
Person SpecificationEducation and Qualifications
  • Registered professional RN/AHP
  • Educated to degree level or equivalent qualification/training and experience
  • Leadership/management course or equivalent experience
  • Willing to undertake further studies in relevant subject
Experience and Knowledge
  • Extensive post-qualifying experience of working autonomously at a specialist level delivering effective patient-focused care to vulnerable adults with complex presentations.
  • Experience of audit, research, and evidence-based practice.
  • Experience in delivering change.
  • Extensive experience of treating people with a range of physical needs.
  • Supports self-care, self-management, and enabling independence.
  • Previous experience of student supervision and clinical education at undergraduate level.
  • Well-developed professional leadership skills.
