They are looking Social Worker to join their Adults Team. What will your responsibilities be? In this role, you will be recording all activities and utilizing the department’s Eclipse information system. You will participate in supervision to allow for the monitoring and development of your performance. You will undertake training as required and coordinate assessments for adults and young people entering Adult Services using Personal Outcome Planning tools, in partnership with service users, carers, and colleagues from other agencies. Additionally, you will develop care packages in collaboration with these partners.
