Adult Social Worker - Urgent and Emergency Care Team£27.40 per hourNottinghamshireFull time, locumHybrid 2 days on siteAbout the role:The Urgent and Emergency Care Teams follow the Discharge to Assess Model. Discharge to assess supports people to leave hospital, when it is safe and appropriate to do so. Our teams work as part of an integrated multidisciplinary transfer of care hub based within the hospitals to plan supported discharges with short term social care support as needed. Once discharged, the goal is for the person to continue their recovery, rehabilitation, and reablement, where we will then support with assessments to promote the individual’s independence and well-being. The council support strength-based practice and we aim to make a real difference to the lives of the people we serve. We are looking for dynamic, skilled and highly motivated Social Workers to join us with a passion for social care and promoting independence. Please note the location of this vacancy will be Bassetlaw Hospital (S81 0BD) and Retford Hospital (DN22 7XF) and therefore there will be an expectation that you cover all sites. There will be an expectation to be on site x 2 days per week.Urgent & Emergency Care Team (UEC NORTH) - Based at Bassetlaw Hospital and Retford Hospital. Maintain a complex caseload and be responsible for the assessment, support planning and review of complex individual needs and then initiate and co-ordinate a range of outcomes to meet them that promote independence. Undertake assessments using a range of health and social care tools, for example, contributing to continuing healthcare assessments. Be responsible for the identification of potential Reablement opportunities and provide access to those services as required. Promote a Strength Based Approach which embeds the ethos of promoting independence and wellbeing at every opportunity, ensures people can assess their own needs, risks and uncertainties to meet their short- and long-term goals, delaying the need for long term care and support. Identify community and other natural support resources, maximising individual’s assets using preventative/universal services and other funding sources, in line with the Adult Care Strategy. Monitor and review ongoing service provision, ensuring all parties involved are fully coordinated, with a focus on wellbeing and outcomes, and that all alternative solutions have been considered. Liaise and negotiate with local providers and support networks to deliver better outcomes. Provide professional detailed information, advice and support to people and their carers, staff and colleagues, by building positive relationships and partnerships. To undertake safeguarding work in the role of Safeguarding Officer.Undertake the role of Approved Adult Mental Health Practitioner (AMHP) or Best Interests Assessor (BIA) or Approved Mental Capacity Professional (ACMP), when trained and approved. Contribute to practice and service development including advising, mentoring and assisting less experienced staff.Organise and manage your workload independently, whilst continuously developing skills and experience, under supervision and guidance of the Advanced Social Work Practitioner and/or Team Manager.
