Role Overview: As a Community Nurse, you will play a crucial role in delivering holistic care to patients in their homes and community settings. Your responsibilities will include assessing patient needs, creating care plans, administering treatments, and collaborating with a multidisciplinary team to ensure the best possible outcomes for patients. Your expertise and support will be essential in promoting health, preventing illness, and facilitating patient independence. Key Responsibilities: Conducting comprehensive assessments of patients’ health needs and developing personalised care plans. Administering medications, treatments, wound care, and other nursing interventions in patients’ homes. Educating patients and their families on health management, disease prevention, and treatment options. Collaborating with physicians, therapists, social workers, and other healthcare professionals to provide coordinated care. Monitoring patients’ progress and adjusting care plans as necessary. Ensuring accurate and timely documentation of patient interactions and interventions. Participating in community outreach programs and health promotion initiatives. Qualifications: Valid NMC Registration Experience in community nursing, home health, or a similar setting is preferred. Strong clinical assessment and critical thinking skills. Excellent communication and interpersonal abilities. Ability to work autonomously and within a team. Compassion, empathy, and a patient-centered approach.
