Collaborating with physicians, therapists, social workers, and other healthcare professionals to provide coordinated care. Monitoring patients’ progress and adjusting care plans as necessary. Ensuring accurate and timely documentation of patient interactions and interventions. Participating in community outreach programs and health promotion initiatives. Qualifications: Valid NMC Registration Experience in community nursing, home health, or a similar setting is preferred. Strong clinical assessment and critical thinking skills. Excellent communication and interpersonal abilities. Ability to work autonomously and within a team. Compassion, empathy, and a patient-centered approach.
