Job Title: Qualified Social Worker – Adult Social Worker Hospital TeamLocation: SunderlandRate: Up to £154.99 per day An exciting job role has recently become available at Sunderland City Council. They are looking for a new member for their Adult Social Worker Hospital Team. What will your responsibilities be? As an adult social worker in a hospital team, you will conduct comprehensive assessments, develop and implement care plans, provide emotional support, and coordinate resources for patients. You will assist with discharge planning, advocate for patients’ rights, and maintain accurate documentation. You must always identify areas of need or risk of harm and ensure appropriate safeguarding measures are in place. Additionally, you need Social Work England registration, eligibility to work in the UK, a full UK driving license, post-qualified experience, and a degree or equivalent qualification in Social Work.
