What will your responsibilities be? The team is responsible for: Responding to safeguarding concerns and crisis situations. Conducting outcome-focused assessments and developing Care & Support Plans. Promoting wellbeing through the Wellbeing Matters Service. Addressing urgent equipment and manual handling needs. Reassessing individuals whose needs have changed.
