CPL uk are looking for an experienced Band 6 Community Mental Health Nurse/Social Worker to work in the Essex, Canvey Island area. You will be expected to work per week.

You will be responsible for:
Assessing, planning implementation and evaluation of the evidence based care required, including health promotion for a named service user/group.

coordinating the care and assessment under the Care Programme Approach of a designated caseload.
Responsible for ensuring that the requirements of the Mental Health Act 1983 are observed, adhered to and implemented.
You will be expected to work collaboratively with key partner organisations, service users, carers, clinicians and other practitioners within the multi-disciplinary team.

PAY RATES: £26-£34 p/h (rates can be escalated for the right candidate).
REQUIREMENTS:
Registered with NMC.
Experience within a community setting.
Completions of all required degrees and diplomas.
