Role: Medicines Optimisation Pharmacist Banding: Band 7 Location: London Start Date: ASAP  Duration: Ongoing Working Hours: Full time, 37.5 hours per week (Monday to Friday, 9:00am – 5:00pm with a 30-minute lunch break) Rate: £28 – £34 per hour About the role: The Pharmacy team at Pulse is seeking a dedicated and experienced Medicines Optimisation Pharmacist to join our team. This role involves working across various community hospitals within the CLCH Trust, focusing on the care of the elderly and rehabilitation wards. Duties and Responsibilities: Conduct Medicines Reconciliation on admission and discharge. Perform Structured Medication Reviews (SMRs). Provide patient counselling and participate in audits. Assist with service improvement initiatives. Communicate effectively both verbally and in writing. Attend Multi-Disciplinary Team (MDT) meetings. Conduct self-administration assessments and answer medication-related queries. Engage in non-clinical work such as updating SOPs, guidelines, writing reports, and supporting the wider Medicines Management Team. Qualifications and Experience: Essential: Up to date GPhC registration. Master’s degree in pharmacy or equivalent. Hospital experience in care of the elderly, rehabilitation, or acute settings. Flexibility to work across multiple sites (no same-day travel required). Car not essential. Desirable: Strong communication and interpersonal skills.Ability to work independently and as part of a multidisciplinary team.Attention to detail and excellent organizational skills.Strong organizational and time-management abilities.Excellent communication and interpersonal skills.
