Job Title: Social Worker – Adult Community TeamLocation: TamesideRate: £35 per hour An exciting job role has recently become available in Tameside. They are looking for a new member for their Adult Community team. What will your responsibilities be? You will work alongside the Community Mental Health Teams to provide social care to individuals with moderate to severe mental health problems. Your duties will include supporting the council’s obligations under the Care Act, completing assessments, funding applications, COPdols, 21a challenges, and other statutory responsibilities. Additionally, you will provide mental health support to residents and assist children transitioning from children’s services to adult services. You should have the ability to work from the office Monday to Friday and visit service users in their homes or supported accommodation .
