Job Title: Social Worker – Transfer of Care TeamLocation: CambridgeshireRate: £38 per hourAn exciting job role has recently become available at the Cambridgeshire. They are looking for a new member for their Transfer of CareTeam. What will your responsibilities be?In this role, you will assess individuals in hospitals for care packages or care home placements under the Care Act, conduct Mental Capacity Assessments for care and support, and collaborate with multidisciplinary teams to create care plans. You will engage in carers conversations and facilitate safeguarding discussions, ensuring a personal approach throughout.
