Job Title: Senior Adult Social Worker – Hospital Discharge TeamLocation: Kingston Upon ThamesRate: £36 per hour Liquid Personnel is currently recruiting for a Senior Adult Social Worker to join a busy Hospital Discharge Team based in Kingston Upon Thames. What will your responsibilities be? As a Senior Adult Social Worker in a hospital discharge setting, you will be responsible for assessing patients’ needs, developing and coordinating comprehensive discharge plans, providing counseling and support, educating patients and families about available resources, advocating for patients’ rights, documenting interactions, visiting care homes to complete Care Act Assessments after the D2A period, and collaborating with the healthcare team to ensure smooth transitions from hospital to home or other care facilities. Your role is crucial in ensuring patients receive the necessary support and resources for continued care.
