Your Role and Responsibilities
Complex Case Management: Work as a complex case manager with direct supervision, providing individualised care to patients with significant health and social needs. Use advanced nursing assessment skills to manage complex cases within the community.
Integration and Collaboration: Play a crucial role in integrating services by working closely with GPs, social services, and third-sector organisations. You’ll deliver case management and support clinical staff while mentoring and performing assessments for patients who face barriers to accessing healthcare.
Community Outreach: Engage in outreach activities, including community events and initiatives, to address the social determinants of health and promote healthcare accessibility for the most vulnerable populations. Support the delivery of healthcare closer to home and within community settings.
Flexible Schedule: Be prepared to work evenings and weekends as part of your regular schedule, ensuring we meet the needs of our diverse community.
About You:
We’re looking for a Registered Mental Health Practitioner with at least two years of community experience managing complex caseloads. You should be confident working autonomously and possess advanced assessment and case management skills. Your ability to liaise with GPs, other professionals, and various agencies will be essential in delivering comprehensive, individualized care to our patients.

Essential Criteria
- Registered Mental Health Nurse (RMN) with current UK NMC registration.
- Degree or Diploma in Adult Nursing
- Evidence of completed leadership courses or training.
- Training on conducting effective appraisals.
- Qualification in Mentorship or Student Supervision (desirable)
- Training in Psychosocial Interventions (desirable)
Knowledge and Experience
Essential Criteria
- Proven experience delivering healthcare to patients with complex needs or hard-to-reach populations in the community
- Broad theoretical knowledge and practical experience across various clinical areas in secondary and primary care
- Experience in assessing, planning, and implementing comprehensive care plans
- Demonstrated experience providing evidence-based care within the community setting
- Recent experience liaising with and negotiating outcomes with system partners to ensure quality patient care
- Ability to manage and organise team caseloads effectively
- Experience mentoring and teaching junior staff, including conducting appraisals
- Knowledge and experience of working to address Health Inequalities
- Experience managing long-term conditions in a community context
- Experience in multi-agency partnership working to deliver seamless, proactive community services
- Evidence of implementing research-based practice and leading service innovation
- Flexibility and a proactive approach to change and service improvements
- Recent examples demonstrating experience in service change management and innovation
- Proficient in using various clinical and IT systems
- Experience collaborating with multidisciplinary teams and system partners across organisational boundaries
- Working knowledge of national health and social care policies, procedures, and their practical application
- Experience in quality and performance management (desirable)
- Project management qualification and experience (desirable)

- In-depth knowledge of health and social care policies and their community application
- Understanding of Clinical Governance and evidence-based practice
- Experience with clinical audits, research, and implementing service innovations
- Effective communication and strong IT skills and excellent numeracy skills
- Demonstrated leadership skills with the ability to inspire and motivate multidisciplinary teams
- Knowledge of clinical supervision benefits and commitment to equal opportunities
- Understanding and adherence to the NMC Code of Professional Conduct
- Ability to handle complex situations and resolve conflicts effectively
- Up-to-date understanding of safeguarding practices
- Full, valid UK driving license with access to a car for business purposes (unless exempt under the Equality Act 2010)
