Are you a passionate and experienced mental health professional looking for your next challenge? We have an exciting opportunity for a Band 6 Care Co-ordinator to join our Community Mental Health Team (CHMT) in the Humber area.

About the Role:

As a Band 6 Care Co-ordinator, you will play a pivotal role in providing high-quality, person-centred care to individuals with complex mental health needs. You will manage a caseload, working collaboratively with service users, families, and other professionals to ensure effective care planning and risk management.

Key Details:

Pay Rate: £25-£28 per hour, depending on experience.
Hours: Full-Time, Monday to Friday, 9:00 AM – 5:00 PM (On-Site Only).
Location: Humber Area – Free Parking Available.
Start Date: December 2024 – January 2025.
Contract Length: 3-6 months, ongoing.

Key Responsibilities:

Developing and implementing comprehensive care plans.
Conducting risk assessments and ensuring appropriate interventions are in place.
Coordinating with multidisciplinary teams to provide seamless and integrated care.
Supporting service users in achieving their recovery goals and maintaining their mental well-being.
Ensuring timely documentation and adherence to NHS guidelines.

Requirements:

Professional Qualification as a Registered Mental Health Nurse (RMN), Social Worker, or Occupational Therapist (OT).
Experience in Mental Health: A proven track record in community mental health services.
Skills: Strong organizational, communication, and interpersonal skills.
Registration: Current registration with the NMC, HCPC, or relevant professional body.
