Please see attached list of organisations.An exciting opportunity has arisen at Barnsley Place within the South Yorkshire ICB for a DSR / C(E)TR Co-ordinator to lead on the All Age Dynamic Support Register, co-ordinate C(E)TRs, collate data for Clinical Assurance Platform and provide administrative support for the Complex Case Manager We are looking for a highly motivated, approachable, dependable and enthusiastic individual to undertake this role.
This is a great opportunity to take a lead in DSR / C(E)TR Co-ordination, making sure colleagues are up to date with relevant legislations and working with different services to ensure children, young people and adults with Learning Disabilities and / or Autism have the same outcomes as those without.
South Yorkshire ICB covers Barnsley, Sheffield, Rotherham and Doncaster and you may be required to work across all sites and travel to Care and Treatment Reviews countrywide.

Interview date: 12/12/2024 Main duties of the job To work in partnership with and ensure continued collaboration between education, health, social care, and voluntary sector partner agencies to provide people with a learning disability and autistic people with appropriate support and care at the right time so that they can lead the lives they want to and meet their ambitions and aspirations; and can stay safely and healthily in the community or return to this as soon as possible.
To collaborate with the local authority and specialist health services in line with contractual arrangements, to provide relevant information, reports, and advice to meet the needs of children, young people and adults with Learning Disabilities and / or Autism.

The aim of the DSR is to bring a person-centred and individualised approach to ensuring that the care and treatment and differing support needs of the person and their families are met, and that barriers to progress are challenged, overcome, and analysed, options compared and reviewed to agree the best way forward.
The DSR Co-ordinator would be responsible for bringing relevant professionals together to collectively discuss and agree a plan to minimise risk and promote wellbeing.
The post holder is not responsible for making any care or clinical decisions but will gather relevant information onto a central register.
The post holder will: Chair the Barnsley Dynamic Support Register update meetings from which will populate/update information onto the Dynamic Support Register (DSR) from information provided by key workers across various agencies and provide data to relevant Department/Agencies Be responsible for horizon scanning and keeping up to date with national guidance and embedding this into systems and processes to ensure a robust Dynamic Support Register to support the needs of the local population.
Provide education, planning and deliver training around DSR processes supported by a standard operating procedure for learning disability and/or autism services across Barnsley place.
Be responsible for providing data and information to meet national reporting requirements and in support of the Transforming Care Program via a monthly monitoring and assurance panels which are linked with C(E)TR delivery.
Be responsible for co-ordinating C(E)TRs for any person who meets the criteria for a review this will include community-based reviews and inpatient reviews.
Be responsible for completing and distributing the KLOE document following each C(E)TR, within prescribed timescales.
