Job summary We are looking for an experienced individual to support service users requiring support from the Community Mental Health Team (CMHT).
The role is to take on care co-ordination for a group of service users who are struggling with severe and enduring mental illness, requiring the extra support and care from a care co-ordinator for individualised care.
You will be working as a MDT group to give excellent care.The Winchester Adult CMHT is looking for an enthusiastic, dedicated, and talented professional to join the team.
Previous experience at a band 5 will be essential -- we welcome applications from ward staff who are now keen to work more sociable hours.

Main duties of the job To be an active member of the multi-disciplinary team supporting the assessment and care planning for our service users.To act as care co-ordinator to an agreed caseload of service users with severe and enduring mental illness.To undertake ongoing assessment, treatment and evaluation for service users known to the serviceTo support the delivery of therapeutic interventions either individually or within a group setting.Support the transformation of the team to deliver local evidence based care.
