Job summary Due to retirement we have an exciting opportunity for an experienced Practice Nurse to join our nursing services team at Eastgate Medical Group.
We are a friendly and supportive practice with sites in both Knaresborough and Harrogate serving 12800 patients.
We are looking for a part-time, enthusiastic, highly motivated, compassionate person to join the team of 3 practice nurses, 1 nurse associate and 2 HCAs.
Main duties of the job Working as part of the practice multidisciplinary team, the post holder will ensure nursing services are delivered effectively to the entitled patient population.
The practice nurse will be responsible for health promotion, chronic disease management, health prevention, as well as supporting the practice team in the delivery of clinical policy and procedure.

Job description Job responsibilities Thefollowing are the core responsibilities of the Practice Nurse.
There may be, onoccasion, a requirement to carry out other tasks.
This will be dependent uponfactors such as workload and staffing levels:To supportthe Infection Prevention Control (IPC) lead and to work to the IPC Policy supportingthe implementation of audits, training, support and evidence of compliance tothe lead nurseTo supportthe production of the IPC annual statement in conjunction with the lead nurseTodevelop, implement and embed an effective care management programme for thefrail and housebound patients registered at the practiceToassess the needs of patients ensuring the provision of healthcare isappropriate, incorporating evidence-based practiceTodevelop, implement and embed health promotion and wellbeing programmesToimplement and evaluate individual treatment plans for chronic disease patientsthat promote health and wellbeingToidentify, manage and support patients at risk of developing long-termconditions, preventing adverse effects to patients healthAsrequired, to provide routine nursing care to patients in accordance withclinical based evidence, NICE and the NSFAsrequired, to diagnose and manage acute and chronic conditionsToreview patient medications to enhance complianceToprovide guidance to patients in the use of prescribed and over-the-countermedications regarding side effects and dosagesTorequest pathology services and process pathology results as requiredTooversee the provision of chronic disease clinics, delivering patient care asnecessary, referring patients to secondary/specialist care as requiredTomaintain accurate clinical records in conjunction with extant legislationToensure read codes are used effectivelyToensure the appropriate maintenance and use of chronic disease registersToensure compliance with the practice chaperone policyToassist GPs with minor surgery when requiredToprioritise health issues and intervene appropriatelyTosupport the team in dealing with clinical emergenciesTorecognise, assess and refer patients presenting with mental health needsToimplement vaccination programmes for adults and childrenTosupport patients in the use of their prescribed medicines or over-the-countermedicines (within own scope of practice)Toliaise with external services/agencies to ensure the patient is supportedappropriately (vulnerable patients etc.
