Job summary A Medicines Care coordinator helps to improve the continuity ofcare around medicines by acting as a point of contact for patients, familiesand professionals.Care coordinators provide extra time, capacity and expertise tosupport individuals in preparing for, or in following-up clinicalconversations they have with primary care professionals.
They will work closelywith the practice prescription teams, PCN Pathway providers and other primarycare professionals within the PCN to ensure coordination and delivery of safeand efficient care with medicines, as possible.
They focus on delivery of comprehensive models of care concerningmedicines to ensure safety and optimise patient outcomes to reflect localpriorities, health inequalities or population health.
Main duties of the job The PCN approach is to deliver integrated care based around theneeds of our local population.
The post holder will play a key role at the heart of our GPPractices, working alongside other PCN colleagues to deliver holistic andcoordinated care to our patient community.
They will work as an integratedmember of the PCN Pharmacy Team.
They will be responsible for the co-ordinationand delivery of specific projects to ensure the achievement of key health andwelfare objectives of Herne Bay PCN.
About us You will working with a team of GPs, nurses, paramedics, physiotherapists and pharmacists in a multi-disciplinary team to deliver care to our population of over 40,000.
You might be seeing patients in the surgeries, doing telephone or video consultations, or doing home visits.
The role is varied and will suit someone keen to develop their skills in primary care and benefit from everything primary care has to offer.
Job description Job responsibilities The post holder will work alongside PCN Pharmacy team members,clinicians and other staff over the practice sites and as part of amulti-disciplinary team.
They will takeresponsibility for an area of work that will contribute to fulfilling therequirements of the Network Contract Directed Enhanced Service (PCN DES).This role is designed to ensure new pathways and/or services areco-ordinated efficiently and effectively, ensuring robust communication withall stakeholders.
The post holder will support and co-ordinate group consultationsacross the PCN.
They will also support the Diabetes MDT clinic, and otherclinics as required by the primary care network and the practices.
Person Specification knowledge/understanding Essential Good knowledge and understanding of Primary Care, Care Homes and the Local System Effective communicator at all levels Desirable Knowledge of QOF and Enhanced Services Knowledge of commonly used or high risk medications and their monitoring requirements Qualifications Essential GCSE grade A* to C (9-4) in English and Maths or equivalent Demonstrable commitment to professional and personal development Willingness to work towards further educational qualifications required for the role.
Desirable NVQ Level 3, Advanced level or equivalent qualifications Skills/Abilities Essential Experience of working within a multi-disciplinary team Experience in using the Microsoft Office suite of applications, including Word, PowerPoint and Outlook A working knowledge and understanding of using Microsoft Excel to work with clinical system data Experience of managing and administering IT systems account Experience of working under pressure and managing varying workloads and changing demands Excellent communication skills with the ability to communicate orally and in written form in a confident, articulate manner Ability to build relationships with others to enable and support collaborative working Desirable Experience of using IT systems such as EMIS Web, Docman and NHS Mail.
Experience Essential Experience of working directly in a medicines related environment, such as GP practice, other healthcare provider, community pharmacy, pharmacy department or team (including unpaid work) Experience of supporting people, their families and carers in a related role (including unpaid work) Experience of partnership/collaborative working and of building relationships across a variety of organisations.
Experience of data collection and providing monitoring information to assess the impact of services Experience of using excel, visio or similar systems.
Full driving license and access to a car.
Ability to travel across the locality on a regular basis, including to visit people in their own homes Desirable Experience of implementing change
