Care Navigator

Job summary We are looking for Care Navigators to join our friendly practice on a part-time basis.
We have fixed term contracts working various shifts on Thursday and Friday.The successful applicants must have previous admin experience and demonstrate excellent communication and organisational skills and previous experience in a healthcare setting would be an advantage.You will be the first point of contact for patients and other contacts, you will book patient appointments, deal with general enquiries, and support the day to day running of the practice.Our opening hours are 08:00-18:30, Monday to Friday, and shifts will be scheduled within these times.
Applicants must be fully flexible to work their shifts as required within these hours.
Please ensure you can commit to this availability before applying, as we are unable to accommodate restrictions outside these times.
There will also be the occasional requirement to work a late night (until 8pm).Applications will be considered as they are received, and interviews will be arranged accordingly.We reserve the right to close the application process if we receive a high number of applications.
Main duties of the job Ensure an effective and efficient reception service is provided to patients and any other visitors to the practice.Deal with all general enquiries, explain procedures and make new and follow-up appointments, either over the telephone or face to face.Initiating contact with and responding to requests from patients, other team members and associated healthcare agencies and providers.Receive and make telephone calls as required.
Divert calls and take messages, ensuring accuracy of detail and prompt appropriate delivery.

We have a daily huddle and regular team meetings.Wells is a beautiful cathedral city with lots to offer, including the Mendip countryside, and within easy reach of Bath, Bristol, and the south coast.You will be joining: -A GMS practice with 8,500 patients, using EMIS, working in a modern, leased building.A friendly, welcoming, and supportive Practice team who are all passionate about Primary Care.A current team of 5 partners, 2 salaried GPs, a paramedic practitioner, nurses, and HCAs, all supported by an accredited Practice Manager and a full admin team.Other team members include and Health Coach, physiotherapists and Clinical Pharmacist.We are a teaching Practice which includes GP Registrars, Medical Students and Apprentice HCAs.A team involved in training and research projects and committed to our local PCN.A practice with Good CQC rating, high patient satisfaction scores, and high QOF achievement.
Job description Job responsibilities The duties and responsibilities to be undertaken by membersof the care navigator team may include any or all of the items in the followinglist.
Duties may be varied from time to time under the direction of thepractice manager, dependent on current and evolving practice workload andstaffing levels:Ensure aneffective and efficient reception service is provided to patients and any othervisitors to the practiceDeal withall general enquiries, explain procedures and make new and follow-upappointments, either over the telephone or face to face.Add allvisit requests onto the duty doctors telephone triage list with a contactnumber and brief reason for visit.Processingand distributing incoming mailComputerdata entry/data allocation and collation; processing and recording informationin accordance with practice proceduresInitiatingcontact with and responding to requests from patients, other team member andassociated healthcare agencies and providersKeepingthe reception area, noticeboards and leaflet dispensers tidy and free fromobstructions and clutterUsingyour own judgment and communication skills, ensure that patients with no priorappointment but who need urgent consultation are seen in a logical andnon-disruptive manner.Explainpractice arrangements and formal requirements to new patients and those seekingtemporary cover, and ensure procedures are completed.Receiveand make telephone calls as required.
Divert calls and take messages, ensuringaccuracy of detail and prompt appropriate delivery.Advisepatients of relevant charges for private (non General Medical Services)services, accept payment and issue receipts for same.Ensurethe kitchen is kept clean and tidy in turn with other staff.Open uppremises at the start of the day when first to arrive, de-activate alarm andmake all necessary preparations to receive patients.When lastto leave at the end of the day, ensure that the building is totally secured,internal lights are off and the alarm activated.Ensurethat all new patients are registered onto the computer system promptly andaccuratelyUndertakeany other additional duties appropriate to the post as requested by thepartners or the practice manager.
