About the Role As a Support Worker, you will provide person-centered support to adults living in a supported housing setting in Shrewsbury.
Your role will focus on empowering customers to build the skills and confidence needed for independent living while working collaboratively to maintain their mental health and wellbeing.
Key Responsibilities: Build positive, professional relationships with customers, promoting trust and engagement.
Support customers in managing their tenancy responsibilities, such as budgeting, maintaining their home, and accessing benefits.
Develop and implement personalised support plans to help customers achieve their goals, focusing on independence and recovery.
Provide practical and emotional support to prevent crisis situations and avoid hospital admissions.
Work with customers to prepare for a successful transition into general needs housing.
Liaise with external agencies, healthcare providers, and other professionals to ensure seamless and effective support.
Keep accurate records and reports, ensuring compliance with policies, procedures, and safeguarding protocols.
About You We're seeking enthusiastic and motivated individuals with a passion for supporting adults facing mental health challenges to thrive and move forward in life.
Essential Skills and Experience: A caring, person-centered approach with excellent communication and interpersonal skills.
Experience in supporting adults with mental health challenges (or transferable skills from a similar role).
A proactive attitude and the ability to work both independently and collaboratively.
Strong organisational skills and attention to detail.
