We are looking for an experienced and highly motivated Senior Clinical Assessor/Practitioner to join our team.

Children and Young People (CYP) referred to Children's Continuing Care service will receive a high-quality service ensuring that the child, family, and carers are at the heart of a personalisation approach.
We work closely with adult Continuing Healthcare to deliver the NHS All Age Continuing Care vision.
This role requires working across Children's Continuing Care, including Children and Young People who are eligible for S117 funding.
Main duties of the job The successful candidate will be working in a collaborative approach to address the child and family's needs; whilst working closely with colleagues from Social Care and Special Education Needs to provide a holistic approach to the support provided.
Though working collaboratively with other key professionals, the successful candidate will need to be able to complete assessments as part of the ICB delivery of the NHS Continuing Care framework, for the children in a full and succinct manner.
These assessments are then presented by the team member to a multi-agency panel where eligibility will be discussed and agreed or declined.
The successful candidate will be part of the clinical team where they will work independently managing their own case load, where they whilst ensuring that they fulfil the ethos of looking after the whole child.
They will have an opportunity to build cross agency relationships.

Job description Job responsibilities All Age Continuing Care covers, Continuing Healthcare and NHS-funded Nursing Care as astatutory duty of the Integrated Care Board for individuals with a primary health need,including Mental Health, Learning Disabilities, Physical Disability, and Fast Age Continuing Care also has Integrated Care Board duties to children and young people(Childrens Continuing Care), joint packages of health and social care (where an individual isnot eligible for Continuing Healthcare but specific needs have been identified through theDecision Support Tool that are beyond the powers of the local authority to meet on its own),personal health budgets, stand-alone health needs, s117 of the Mental Health Act andSpecialist Rehabilitation (Acquired Brain Injury).All Age Continuing Care delivers the patient specific Learning Disabilities and AutismProgramme Integrated Care Board responsibilities.
Person Specification Knowledge, Training and Experience Essential Registered Health or Social Care Professional with current registration with professional body NMC, HCPC or SWE.
including AMHP's and mental health social workers or significant relevant experience (LD/MH/LDAP teams only).
Educated to degree level in relevant subject or equivalent level of experience of working at a similar level in health or social care.
Experience of assessment of health and social care needs - care planning and coordination.
Knowledge and relevant experience working with individuals who are autistic and/or have learning disabilities and/or mental health issues (LD/MH/LDAP team roles).
Knowledge of complex neurological conditions and experience of working with clients with complex brain injuries.
Specific knowledge of the National Service Frameworks and appropriate national policy related to rehabilitation of people with acquired brain injury/ neurological dysfunction (Specialist Rehabilitation / ABI team).
Ability to work effectively under stressful conditions in a rapidly changing environment.
Good knowledge of Mental Capacity Act and relevant legislation (for DOLS team) Experience of implementing and managing Personal Health Budgets.
Desirable Demonstrable experience of care/case management.
Knowledge of and experience of NHS Funded Continuing Health Care criteria - guidance and policies and other legal frameworks as required (Children and Complex Care, s117 Aftercare, Mental Health Act).
Experience of developing care packages.
Experience in communications and stakeholder management.
Effective working knowledge of Microsoft software.
A good understanding of the health and social care environment and roles and responsibilities within it.
Communication and Relationship Skills Essential Clear communicator with excellent writing, report writing and presentation skills; capable of constructing and delivering clear ideas and concepts concisely and accurately for diverse audiences.
Skills for communication on complex matters and difficult situations, requiring persuasion and influence.
Skills for nurturing key relationships and maintaining networks.
