Job summary Fixed term contract/secondment opportunity for 23 monthsThe Haemoglobinopathy CNS is a key role within the Red Cell service, and the post-holder will operate independently, but based within the wider Haematology Clinical Nurse Specialist team, co-ordinating the care of patients throughout their whole life as well as supporting the clinical and research activities of the team.
The post-holder will play an active part in developing the service, supporting and preparing for Peer Review, and ensuring that it is patient-centred, safe, efficient and in line with national standards.If you are considering applying for this post on a secondment basis, please discuss with your Line Manager prior to applying to ascertain whether this could be supported.
Main duties of the job As post holder you will liaise with other hospitals to co-ordinate care of shared patients, disseminating clinical protocols and providing education and support and liaising within the community.
The CNS is a specialist educational resource and will be expected to facilitate the professional development of nurses throughout the Trust and Network as well as training staff in manual exchange.
You will also support the key research activities of the team.
Key working relationships are with all members of the multi-disciplinary team including nursing and medical staff, allied health professionals and support staff.As post holder you will manage and co-ordinate clinical follow-up programmes, working as part of a multi-disciplinary team.
You will act as a key-worker for patients, families and facilitate transition between child, adolescent and adult services.
You will take a leading role in setting up nurse-led clinics in this area.
You will be expected to have an in-depth understanding of the apheresis process and treatment aims.
You will also have an understanding and knowledge of potential medical and psychological needs of children, young people and adults with haemoglobinopathy conditions.

To operate autonomously at an advanced level of clinical practice providing research based expert care and advice to all colleagues and managing a caseload effectively, making the most of available timeTo accept referrals from medical/nursing staff both within and external to the Trust, of newly diagnosed and existing patients To periodically review caseload and to discharge patients appropriately within to divisional guidelinesTo ensure that patients are seen within the recommended time scales and targets where these are in placeTo utilise specialist, skills, underpinned by in depth theoretical knowledge, to facilitate the assessment, planning and implementation of individualised programmes of care for patients presenting with a specialist condition and to evaluate their programmes of care.To assimilate and communicate highly complex, highly sensitive and emotive information regarding patient diagnosis and prognosis, ensuring that patients, relatives and the multidisciplinary team understand and accept the reasons for treatment (or non-treatment) plansTo offer and provide specialist advice through listening and counselling skills.To contribute to service developments and the development of nurse led initiatives in liaison with Senior Nurse/Service Manager/Operational Manager/Assistant Director of Nursing.To contribute toward divisional, Trust and corporate objectives and service plans.To participate in clinical supervision on a monthly basis.To lead on provision of support and supervision for all staff to develop specific immunotherapy knowledge and skills, through opportunities to shadow the specialist teams and to observe practice.To be responsible for acquiring, developing and maintaining agreed competencies and skills, according to level of practice.Act as a role model providing mentorship and/or clinical supervision to other staff/learners supporting them in applying theory to practice.
