If you are dynamic, dedicated and enthusiastic then we want to hear from you.We are a multi disciplinary community team which provide secondary health services to adults with a learning disability.Full time and part time hours will be considered.Please note this is a countywide community post and the successful applicant will be required to travel within the geographical area without relying on public transport.
Main duties of the job As a Senior Support Worker you will be an active and positive member of the Community Learning Disability Team, contributing skills and experience to the clinical assessments and interventions.
Working with the individuals with a learning disability and their families you will monitor, contribute to, and modify the Care Plan in response to need, in order to improve the health outcomes for the person.

We deliver community and hospital-based care to improve people's physical and mental health.
We also provide specialist support to people with dementia or a learning disability.We are a people organisation and people matter to us.
As part of the team, you'll help support the health and wellbeing of the people who live and visit this beautiful part of the UK.Over 4,000 people make up the Trust.
This includes doctors, nurses, therapists, plus admin and support staff.We work in people's homes, in community clinics and bases.
Some staff work from one of our 13 community hospitals.
Our aspiration is to have great people, provide great care, be a great place to work and a great partner.Approximately 568,000 people live here.
A third of people who live in Cornwall are supported by acute hospital services in Devon.
As a result, we also work closely with our partners in Devon.
In the summer, and during other holidays lots of people choose to visit the area.
This increase the numbers of people who use out services.

Person Specification Education/Qualifications and Relevant Experience Essential NVQ Level 3 in Health and Social care or equivalent relevant experience/qualification Previous experience of working with people with a learning disability with complex needs, some of whom exhibit behaviour that challenges, in a variety of settings.
Desirable Experience in a variety of settings.
Experience working with people with complex behavioural and/or mental health needs Skills and Aptitude Essential Excellent communication and record keeping skills.
Ability to recognise and report with accuracy the people with a learning disability needs and/or environment.
