Ouraim is to provide high-quality, patient-centred care in the comfort of theirown homes, reducing hospital admissions & length of stay in hospital &enhancing overall well-being through enhanced care coordination.All Livewell Southwest staff are expected to be able and willing to work across a 7 day service.LWSW may close the job advertisement earlier than the specified deadline if a high number of applications are submitted.
Therefore, we recommend you submit your application at the earliest opportunity.Please note that this role may not be eligible for sponsorship under the Skilled Worker route, please refer to the Direct Gov website for more information with regards to eligibility Main duties of the job As Advanced Care Practitioner, you will play a critical rolein our new multidisciplinary team, covering one or more of the 6 new frailtyvirtual wards that we have planned for our locality.
The post holder will workclosely with consultants, pharmacists, dietitians, and other healthcareprofessionals to ensure seamless, integrated care on the virtual ward, byproviding specialist ACP assessments and interventions for those admitted tothe Frailty Virtual Ward.

Job description Job responsibilities The postholder will and in accordance with Multi-professional framework for advancedclinical practice in England their professional code of practice and legalframeworks such as non-medical prescribing.
Advanced Clinical Practice a levelof practice characterised by a high degree of autonomy and complex decisionmaking.
This is underpinned by a masters level award (L7) that encompasses thefour pillars of clinical practice, leadership and management, education andresearch, with demonstration of core capabilities and area specific clinicalcompetence.Workingautonomously as an Advanced Clinical Practitioner within the clinical team andprovide a high level of expertise to ensure patient-centred clinical care inthe most appropriate clinical setting Exerciseadvanced clinical expertise, levels of judgement, discretion and decisionmaking whilst undertaking the skills of assessment, examination to provide adiagnosis and formulate timely treatment plans within an agreed scope ofpractice Evaluateinvestigations and revise treatment plans according to patient need and assessimpact and outcome.
Assistin the safe referral and discharge of patients with undifferentiated andundiagnosed presentations in any areas of working Provideexpert professional advice to patients, carers and colleagues Monitorand lead improvements to standards of care through; supervision of practice,clinical audit, implementation of evidence based practice, teaching and supportof colleagues and the provision of professional leadership.
Leadthe implementation of new evidence based practice and contribute to thedevelopment of the evidence through research, audit and collaboration withclinical or academic staff.
Contributeto clinical governance by leading quality improvement, audit and researchprojects as part of the multidisciplinary service-wide performance and qualityagenda.
Rolemodel the Organisations values and behaviours, demonstrating a person centredapproach to service delivery and development whilst maintaining a professionalportfolio that supports their scope of practice.4.
SCOPE AND RANGEThepost holder will be an autonomous registered clinician who is confident to workas the only clinician when required and with a consultant/specialty doctor/GPon other occasions.
The Advanced Clinical Practitioner will work within a definedscope of practice and to agreed clinical standards In line with HEE ACPframework (2017) The post holder will provide a high level of expert clinical care,promoting clinical leadership and set a good standard of professional practicewith the organisationThepost holder provides a visible, accessible and authoritative presence withinLivewell services and community settings.Thepost holder will be working across organisations and communicating with a rangeof professionals, patients/clients and their families.Responsibilitieswill include managing whole episodes of care by supporting medical colleagues,history taking, clinical assessment, diagnosis, treatment includingprescribing.
The ACPwill be responsible for ordering and acting on investigations, creatingmanagement plans, onward referral and evaluation of careThe post holder will lead the development of non-medical ledmodels of care across community services, in collaboration withmulti-professional and systems colleagues.5.
MAIN DUTIES/RESPONSIBILITIES The ACP will be based in the specified Team and works within otherrelated working areas across the specified clinical pathway.The post holder will:Proactively support the development of the ACP role within the teamprimarily focusing on the holistic management of patients within the clinicalcaseload and service model.
Focussing on key areas for example (not exhaustive)admission avoidance to positive discharge, reducing length of stay, and deliveringquality patient care.
This could be through community management and inreachwork.
Actively participate in professional leadership and education acrossthe nursing, medical and therapy teams.
Actively participate in and represent the Organisation in the Regionalnetworks Actively participating in developing the service.The post holder will manage own workload within areas of individualcompetency in the service area for example various clinics, diagnoses in orderto choose appropriate evidenced based treatment options; follow up frequency,and referral to other pathways ordischarge.This will include: Utilising advanced clinical reasoning skills and assessment techniquesautonomously in the context of their speciality Interpreting broad policy: Establish the way in which these should beinterpreted and establishing standards.
Acting as a lead specialist within their sphere of expertise.
Working within their professional code of conduct.
Assimilating risk/ benefits and rationalise decision making based onextensive knowledge skills and experience, recognising and acting on potentialgaps in knowledge.
Seeking out advice and support from consultant/medical colleagues whenrequired.

Qualifications Essential Professionally Registered eg Nurse/ AHP / Pharmacist Evidence of MSc Advanced Practice or equivalent (mapped to all outcomes of the Multiprofessional Advanced Practice Framework HEE) Evidence of competence in the trusts core ACP capabilities or willingness to work towards in 6 months Evidence of achievement of appropriate credential or willingness to work towards this in post eg Royal College of Physicians as per the NHS Education England Framework Non-Medical Prescribing (supported as part of AP pathway, there will be very few exclusions to this eg OT APs are excluded due to statute) Desirable HEE Digital Badge Post Grad Teaching qualification or equivalent Management qualification/leadership qualification Experience Essential Experience of Innovation and change management Experience in applying research findings to practice Understanding of health promotion and counselling Experience of providing clinical supervision in practice Desirable Extensive experience in caring for patients across a wide variety of clinical settings
