Job summary This role will be part of the Stokewood Surgery team and the post holder will play a crucial role in delivering high-quality patient care.As a key member of the Primary Care Network (PCN) anticipatory care team, you will work autonomously to provide a holistic approach, encompassing assessment, diagnosis, treatment, and signposting.
Main duties of the job A paramedic in general practice plays a critical role in supporting primary care teams and improving patient access to care.
Their main duties typically include:Patient Assessment and Management: Conducting initial assessments of patients, diagnosing common conditions, and providing treatment within their scope of practice.
Urgent and Emergency Care: Responding to urgent cases within the practice, such as managing acute illnesses and supporting emergency care as needed.
Home Visits: Performing home visits to assess and treat patients unable to attend the practice.
Chronic Disease Management: Assisting with monitoring and managing patients with long-term conditions such as diabetes, asthma, or hypertension.
Health Education and Advice: Offering guidance on health promotion, disease prevention, and lifestyle modifications.
Referral and Coordination: Referring patients to appropriate specialists or community services and ensuring continuity of care.
Administrative Duties: Maintaining accurate patient records, completing documentation, and contributing to audits and practice reviews.
Paramedics in this role work collaboratively with general practitioners, nurses, and other healthcare professionals to enhance patient outcomes and support the smooth functioning of the practice.
About us Winchester Rural South Primary Care Network (PCN) comprises four GP practices: Stokewood, Twyford, Wickham, and Bishops Waltham surgeries.
Together, they provide personalised, high-quality healthcare services to the rural communities in North and Mid Hamphire.Stokewood Surgery, part of this PCN, is located in Fair Oak, Eastleigh.
It offers general practice services and contributes to the PCN's collective efforts to improve healthcare access and delivery in the area Job description Job responsibilities Work as part of a MDT within the PCN Assess and triage patients, including same day triage, and as appropriate provide definitive treatment (including prescribing medications following policy, patient group directives, NICE (national) and local clinical guidelines and local care pathways) or make necessary referrals to other members of the primary care team.
Advise patients on general healthcare and promote self-management where appropriate, including signposting patients to the PCNs social prescribing service, and where appropriate, other community or voluntary service.
Be able to: Perform specialist health checks and reviews within their scope of practice and in line with local and national guidance.
Perform and interpret ECGs.
Perform investigatory procedures as required.
Undertake the collection of pathological specimens including intravenous blood samples, swabs, and other samples within their scope of practice, and within line of local and national guidance.
Support the delivery of anticipatory care plans and lead certain community services (, monitoring blood pressure and diabetes risk of patients living in sheltered housing).
Provide an alternative model to urgent and same day GP home visit for the network and clinical audits.
Communicate at all levels across organisations ensuring that an effective, person-centred service is delivered.
Communicate proactively and effectively with all colleagues across the multi-disciplinary team, attending and contributing to meetings as required.
Maintain accurate and contemporaneous health records appropriate to the consultation, ensuring accurate completion of all necessary documentation associated with patient health care and registration with the practice.
Communicate effectively with patients, and where appropriate family members and their carers, where applicable, complex, and sensitive information regarding their physical health needs, results, findings, and treatment choices.
Duties may vary depending on the practice need, but will algin in accordance to the Primary Care Network Directed Enhanced Service contract Person Specification Qualifications Essential Active and relevant professional registration with HCPC BSc or above in Paramedic Science or equivalent degree MSc in Advanced Clinical Practice or equivalent Independent Non-Medical Prescribing Qualification Evidence of relevant CPD Knowledge of primary and community healthcare Advanced clinical practice, prescribing and examination skills Excellent communication skills Basic IT Skills Evidence of commitment to own personal and professional development Ability to manage and prioritise workload Ability to work as part of an integrated multi-disciplinary team Ability to work flexibly to meet the needs of the Primary Care Network Understanding health education/promotion Understanding of Clinical Governance and its significance and impact upon practice Awareness of equality and valuing diversity principles Personal Requirements UK driving licence, with exclusive access to a vehicle Desirable Experience or working in a primary or urgent care setting Experience of working in a GP practice MSc in Advanced Clinical Practice or equivalent Independent Non-Medical Prescribing Qualification Knowledge/use of primary care IT software EMIS
