We are seeking a newenthusiastic team to join this exciting fixed term opportunity.
Main duties of the job We are seeking motivated GPcandidates to work in our newly setup Acute hub.
You will be seeing patientswho have been triaged from across our 4 PCN practices at a centralisedlocation.
These patients will have one acute issue requiring same day assessmentand management.
Complex patients pregnant patients or those on palliativecare register would not be eligible to be seen in the acute hub.
The successfulcandidates may also be required to provide on the day clinical supervision toallied health professionals.The acute hub will initially operatebetween 9am and with the majority of patients being seen in face to faceconsultations.
The acute hub patient list and consultation notes will berecorded on System One.
The vision for the acute hub is that it will be adminfree no referrals, no tasks and no home visits.
The successful candidate willhave the opportunity to help us shape this exciting new service alongside theAPEX PCN team.
About us APEX PCN is located in West Lincoln and incorporates 4 practices Birchwood Medical Practice, Boultham Park Medical Centre, Richmond MedicalCentre and Woodlands Medical Practice.
We have a population of 46,000 with apredominantly younger demographic.
The PCN is supported by a strong ARRS team.This includes pharmacists- engaged in supporting practices in their dailyworkload while also focussing on the DES requirements.
We have an effective FCPteam supporting practices for MSK care and a well-developed EHCH and Frailty MDTto support care homes.
Our practices work collaboratively to achieve our IIFtargets and PCN DES requirements.
We are now looking at forming strategicpartnerships with our other Health and Care colleagues to effectively addressthe health needs of our population.
Job description Job responsibilities Thepost holder is GP who, acting within their professional boundaries, willprovide care for the presenting patient from initial history taking, clinicalassessment, diagnosis, treatment and evaluation of care.
They will demonstratesafe, clinical decision-making and expert care, including assessment anddiagnostic skills, for patients within the PCN.
The post holder willdemonstrate critical thinking in the clinical decision-making process.
Theywill work collaboratively with the general practice team to meet the needs of patients,supporting the delivery of policy and procedures, and providing nurseleadership as required.
The Candidate will: Manage undifferentiated undiagnosed condition andidentify red flags and underlying serious pathology and take appropriateaction.
Use complex decision makingto inform the diagnosis, investigation, complete management of episodes of carewithin a broad scope of practice.
Actively take a personalised care approach andpopulation centred care approach to enable shared decision making with thepresenting person.
Complete the relevant training in order to providemulti-professional clinical practice and CPD supervision to other roles withinprimary care, for example Advanced nurse practitioners and the personalisedcare roles.
Utilise and demonstrate sensitive communicationstyles, to ensure patients are fully informed and consent to treatment.
Communicate effectively with patients and carers,recognising the need for alternative methods of communication to overcomedifferent levels of understanding, cultural background and preferred ways ofcommunicating.
Anticipate barriers to communication and takeaction to improve communication.
Maintain effective communication within the urgentcare service and with GP practices and external stakeholders.
Act as an advocate for patients and colleagues.
Ensure awareness of sources of support andguidance ( PALS) and provide information in an acceptable format to allpatients, recognising any difficulties and referring where appropriate.The post holder will maintain the security ofsensitive personal and other confidential information and will work within allrelevant Information Governance policies reliably to working practice.Additionally, they will be expected to follow secure operating procedures forhandling information in all formats, including verbal, written and that heldelectronically.
The post holder will have the appropriate level ofchild protection and adult safeguarding knowledge, skills and practice requiredfor the post; be aware of and comply with child protection and adultsafeguarding policies and procedures.
This post will be subject to a DBS (Disclosure andBarring Service) Check In addition to observing the host Practices rulesand procedures, which all staff are required to observe and follow, the postholder is also required to follow general policies and procedures that apply tothe employment relationship.

Person Specification Qualifications Essential MRCGP / Relevant clinical qualification A medical practitioner whose name is included in the General Practitioner Register under article 10 of the General and Specialist Medical Practice Order 2003 Not subject to suspension under section 41A of the Medical Act 1983 Qualified General Practitioner (completed certificate of Completion of Training CCT) Currently on a performers list and not suspended from that list or from the medical register Experience Essential Experience In practice as a General Practitioner/Locum Practitioner Experience or additional training in clinical supervision Knowledge of quality improvement methodologies Excellent clinical assessment and decision-making skills Strong communication and interpersonal skills Ability to work under pressure and manage multiple priorities
