Job summary We are looking for an enthusiastic, honest, self-motivated colleague who is able to provide high quality person centred care to patients, their families and carers within the Specialist Dementia and Frailty ServiceMulti-Disciplinary Team (MDT) in a range of settings in the West Essex Area.
Main duties of the job The SDFS team provide a comprehensive assessment, diagnostic and treatment pathway supporting patients of any age with dementia as well as older adults over 75 years of age, who have complex functional mental health needs with an associated age related frailty.
We work closely with our EPUT community specialist practitioners and Essex County Council colleagues, local organizations as well as the voluntary sector to support the holistic needs of the patient with a person centred collaborative approach.
The successful candidate will be trustworthy, positive about change and will be passionate about the services we provide.
The successful candidate will be able to work independently and autonomously, as well as part of a team therefore will need excellent verbal and written communication skills and will have excellent interpersonal skills and a zest for delivering the highest level of patient care and service delivery successful candidate will be able to adhere to the trust core values of being compassionate, open and empowering.

RMN Part 3 Mentorship preparation Evidence of continual professional development Meet the requirements for revalidation as set out by the NMC High standard of education Desirable Post graduate qualification or commitment to work towards this Non-medical prescribing qualification Knowledge Essential Understanding of / and the ability to conduct assessments and review of care in line with the requirements of the mental health care clusters Understanding and the ability to support the team to meet their key performance indicators and contractual targets Experience in the performance management and supervision of lower banded staff Excellent IT skills Word / Excel, PowerPoint and Outlook Experience of using an electronic information system Experience of and participation in audit and research Knowledge and understanding of health legislation, principles of CPA and Care Management, Mental Capacity Act, Mental Health Act 1983 and safeguarding, the NHS Constitution, and risk assessment / management Knowledge of current best practice and models within nursing practice in mental health The ability to devise comprehensive care plans that incorporate effective, individualised and evidenced based therapy interventions Understanding of the nature of mental health conditions and how they can affect people's day to day life and impact upon recovery Commitment to ongoing learning through CPD for self and others within operational area Skills/Experience Essential Literate in IT/Computer Skills Proven experience of working in a health, social care or community setting as part of a multi- disciplinary team Experience of dealing with confidential and sensitive information Proven experience of working with people with mental health conditions and vulnerable service users Proven experience of conducting and contributing to ongoing assessments Ability to conduct risk assessments of the individual in the service user environment Proven experience of medicines management in line with NMC Standards and Trust policies and procedures Experience in providing psycho- social interventions Demonstrate an understanding of the recovery principles Ability to manage and coordinate mulit-disciplinary meetings referral / review / business / handover Ability to work with Clinical Manager in the development and implementation of operational / Trust policies Deliver training to colleagues and lower banded staff Ability to critically analyse and review research findings and reports Assess and treat own caseload of service users in accordance with Trust clinical pathways Undertake physical health checks of services users and act on the findings appropriately Undertake a range of clinical duties providing practical support and guidance to service users Demonstrate an understanding of the Trust Core Values Understand the need for compassion in working with colleagues, servicer users and families Support families and carers within the boundaries of their role and NMC professional code of conduct Capable of working under pressure and meet deadlines where required Act as key worker to a group of service users Take on the role of lead practitioner Trust policy Requirement to undertake carers assessments Undertake, supervise and sign off nursing needs assessments Lead and facilitate group work Excellent interpersonal and communication skills, both written and verbal, telephone manner including liaison with outside agencies Record all service user related information on Trust electronic records system Lead/chair professionals meetings and reviews Ability to report back at professional meetings / reviews on an individual / groups performance Produce reports for managers appeal / renewal hearings and mental health review tribunals Supervise the work of nursing students and support workers Excellent organisational skills, ability to prioritise own workload and that of others Ability to adapt to change when required Carry out /
