The Team consists ofCommunity Nurses, Physiotherapists, Occupational Therapists, HealthcareAssistants and admin staff with the wider multidisciplinary team.
We arelooking for a highly motivated, flexible team player with a strong, qualitydriven patient focus and an ability to work through change to embed new ways ofdelivering care.

Main duties of the job A typical day as community nurse may start with greeting adiabetic patient and administering their insulin, followed by supportingpatients at the end of their life to ensure gold standard care is delivered.You may then visit a few patients to dress a leg ulcer or post op wound,followed by changing a catheter to flushing a patients central line.
You may becalled out to an urgent care response visit where a patient needs to be seenand assessed within 2 hours.
Following a list of visits you will have teamhandover, this gives you the opportunity to discuss complex situations with yourcolleagues and undertake clinical supervision.
No day is the same within thecommunity and each day is as rewarding as the next, whether youve healed a legulcer or given a patient the best possible care at the end of their life.

This will be achieved through acombination of hands on clinical skills and knowledge to treat, educate andsupport patients on a designated caseload and by working with patients andtheir carers to assess, plan and implement appropriate packages of care.Regularly take responsibility for the caseload in theabsence of the Senior Community Nurse.

Person Specification Knowledge Essential Variety of evidence based clinical skills Confidence to teach student nurses, trainee nursing associates, patients, carers and members of the multidisciplinary team Understanding of evidence-based nursing, multidisciplinary working, clinical governance Desirable Competence in male and suprapubic catheterisation Venepuncture and IV therapy Cannulation Management of syringe drivers Experience Essential Experience gained during Pre-Registration training Desirable 2 years post registration experience Community nursing experience Experience of management of caseloads Additional Criteria Essential Self-motivated and able to motivate others Team player Able to cope under pressure Compassion and empathy skills Able to meet the travel requirements of the role Desirable Willingness to undertake further training Qualifications Essential Registered Nurse (RN1) I understand that I will be asked to provide original certificates to verify my qualifications at point of offer.
