Job summary This role as a Team Leader at the Grove Park Care Home in Leeds offers the opportunity to lead a dedicated team in delivering high-quality, person-centred care to residents.
You will be responsible for supporting the home management, implementing care plans, monitoring resident wellbeing, and ensuring a safe, caring, and inclusive environment.
Main duties of the job As a Team Leader, you will focus on supporting the home management team in delivering high-quality care with a difference.
Your responsibilities will include leading in the person-centred care planning process, assisting with resident discharges, monitoring resident wellbeing, and practicing safe systems of work.
You will need to have an NVQ 3 or equivalent in care, two years' experience in a senior position, and the ability to communicate effectively and lead staff.

If this sounds like the place for you, wed love to hear from youABOUT THE ROLEYour focus asTeam Leaderwill be tosupport thehome management team in delivering high-quality care with a difference by providing a safe, caring, innovative, open, supportive, and inclusive environment for residents, visitors,and staff.Other responsibilitieswillinclude: Leadingin the person-centred care planning processby implementing,monitoring,and undertaking monthly evaluations, completing care plan audits, developing action plans, andbeing responsible forcompletingthe action plans.
Assistingin the smooth transition of any discharges from the home, including documentation, medication, andkeeping theresidents'next of kin fully informed, whereappropriate.
Leadingwithmonitoringthewell-being of each resident, being aware of any changeimpactingcare and care planning andensuring that changing needs are met and written records are amended to reflect any variation.
Practicing safe systems of work across a range of tasks and moving and handling of residents, by assessing risks and having due regard for personal safety and the safety of residents, visitors, and staff.ABOUT YOUTosucceed in your application, you will live our values of caring, supportive, honest,respectful,and accountable in allyou do.Our ideal candidatemust: Have anNVQ 3 or equivalent in care for care homes.
Twoyears experienceworking in a senior position with the residentgroup.
