Job summary An exciting opportunity has arisen for an Advanced NursePractitioner (ANP) to join Crane Valley Primary Care Network.
The post holderwill be employed by Crane Valley and will work for Verwood Surgery.
We arelooking for a collaborative and innovative ANP to work closely with clinicalteams to deliver patient care.
Main duties of the job The post-holder will: Work independently to offer direct access for the general practice population via triage clinics, conducting appointments either in person or via telephone.
Serve as the initial point of contact within the Practice for patients presenting with undiagnosed or undifferentiated health issues, utilising skills in history taking, physical examination, problem-solving, and clinical decision-making to establish a diagnosis and develop a management plan, including home visits if needed.
Participate in a rota to support the triage of daily patient requests, making immediate decisions and redirecting patients to the most suitable clinician within the appropriate timeframe.
May include working some extended hours.
Utilize advanced professional accountability, autonomy, and judgment when assessing and treating patients with diverse situations and conditions.
Assist in achieving management team objectives where possible.
Participate in mandatory and internal/external training as required.
Adhere to all policies and protocols in the Staff Handbook and contract, demonstrating behaviours aligned with Verwood Surgery values.
Follow Practice Health and Safety guidelines and report any infringements to the H&S lead.
About us The practice team is made up of 4 Partners, one Salaried GP,four fantastic practice nurses, a surgery frailty worker, PhysiciansAssociate, Phlebotomists, management team and our administration/receptionstaff.
We are a training practice and also regularly have registrars.

They have thefreedom and authority to act and make autonomous decisions in the assessment,diagnosis and treatment of patients.
TheANP provides an opportunity for patients to avoid the need to be referred to aGP, thereby enabling them to receive timely care and negate unnecessary delayin receiving treatment.
ANPsin primary care can develop close, long-term relationships with their patientsand work in partnership with them to achieve optimum health.
They areautonomous in making decisions based on assessment, diagnosis, andinterpretation of test results.
They can independently prescribe appropriatemedication, evaluate, or refer to other specialists if necessary.Primary Key ResponsibilitiesThe following are the core responsibilities of the Advanced Nurse Practitioner.
There may be, on occasion, a requirement to carry out other tasks; this will be dependent upon factors such as workload and staffing levels:Develop, implement and embed health promotion and wellbeing programmesManage patients presenting with a range of acute and chronic medical conditions, providing subject matter expert adviceImplement and evaluate individual specialised treatment plans for chronic disease patientsIdentify, manage and support patients at risk of developing long-term conditions, preventing adverse effects to the patients healthProvide advanced, specialist nursing care to patients as required in accordance with clinical based evidence, NICE and the NSFProvide wound care (ulcer/Doppler, etc.
)Provide specialist clinics such as ear syringing, contraception, etc.Undertake the collection of pathological specimensProvide travel medicine servicesRequest pathology services as necessaryProcess and interpret pathology and other test results as requiredProvide chronic disease clinics, delivering patient care as necessary, referring patients to secondary/specialist care as requiredMaintain accurate clinical records in conjunction with extant legislationEnsure SNOMED CT codes are used effectivelyMaintain chronic disease registersDevelop, implement and embed an effective call/recall systemBe responsible for the implementation of an effective immunisation programmeChaperone patients where necessaryPrioritise health issues and intervene appropriatelySupport the team in dealing with clinical emergenciesRecognise, assess and refer patients presenting with mental health needsImplement vaccination programmes for adults and childrenBe an extended and supplementary prescriber, adhering to extant guidanceSupport patients in the use of their prescribed medicines or over the counter medicines (within own scope of practice), reviewing annually as requiredContribute to practice targets (QOF, etc.
), complying with local and regional guidanceLiaise with external services/agencies to ensure the patient is supported appropriately (vulnerable patients, etc.
)Delegate clinical responsibilities appropriately (ensuring safe practice and that the task is within the scope of practice of the individual)Support the clinical team with all safeguarding matters in accordance with local and national policiesUnderstand practice and local policies for substance abuse and addictive behaviour, referring patients appropriatelyDeliver opportunistic health promotion where appropriateSecondary ResponsibilitiesIn addition to the primary responsibilities, the Advanced Nurse Practitioner may be requested to:Act as the audit lead, effectively utilising the audit cycleSupport IPC lead, ensuring compliance, helping to undertake audits as necessaryMonitor and ensure the safe storage, rotation and disposal of medicamentsUndertake overall management of the nursing team, providing guidance when necessary, acting as a mentor to students and newly qualified staffParticipate in local initiatives to enhance service delivery and patient careSupport and participate in shared learning within the organisationDevelop an area of specialist interest, taking the lead within the organisationContinually review clinical practices, responding to national policies and initiatives where appropriateParticipate in the review of significant and near-miss events applying a structured approach , root cause analysis (RCA)Drive the development of nursing services within the organisation, liaising with external agencies and professional organisations as requiredDevelop practice administrative and clinical protocols in line with the needs of the patient and current legislation Person Specification Qualifications Essential Registered Nurse with Nursing and Midwifery Council Post graduate diploma or degree for Advanced Practice Qualification, MSc advanced practice programme or completed the Centres ePortfolio (supported) Route refer to Accredited Programmes - Advanced Practice Qualified Independent Nurse Prescriber on the NMC register Meets NMC revalidation requirements in accordance with the NMC Revalidation booklet Minor illness qualification Desirable Reaching qualification Experience of working as a practice or community nurse Qualified triage nurse
