Job summary Lancashire & South Cumbria Foundation Trust are pleased to be recruiting to the Keyworker Team, who play an integral role for children and young people with ASD and learning difficulties who are in crisis.
The Keyworker Team provides a multi-agency approach to support children, young people and families/carers by delivering comprehensive training to strategic leaders, commissioners and frontline colleagues.
It is a co-ordinated, person-centred and outcome-focused approach to planning and support where children, young people and families/carers require additional and sometimes complex support.
The Designated Keyworker is in high demand, with each Keyworker managing small caseloads.
As an LSCFT team, we work across Lancashire, Blackpool and Fylde Coast, Morecambe Bay, South Cumbria and Blackburn with Darwen.

Main duties of the job The post holder willEnsure co-production and co-design with parent carers and Children and Young People (CYP) who have a learning disability and/or autism as well as colleagues across actively involved with the Dynamic Support Register (DSR) leads in each of the Integrated Care Partnerships (ICPs), allowing for a comprehensive overview of Lancashire & South Cumbria children and young people.
Supervision and support of the Designated Keyworker Development Officers.
Ensure that families are supported directly through embedded processes such as Local Area Emergency Protocol (LAEPs) and Care, Education and Treatment Reviews (CETRs).
Ensure clarity of data through the use of the ICP DSR for the recording and tracking of CYP to ensure organisations can co-ordinate their support for the CYP and family effectively.Develop and maintain a strong, positive relationship with children and young people with a learning disability, autism or both, who are inpatients or at risk of being admitted to hospital, to ensure children, young people and their families get the right support at the right time and that local systems are responsive to meeting their needs in a holistic and joined up way.
To facilitate transition to an identified lead within the multi disciplinary team to enable a step-down process ensuring continuity of the right system of community services and support to meet the child/young person's needs once key working involvement ceases.

Person Specification Education/Qualifications Essential Educated to Degree level in a relevant subject or equivalent level of qualification or significant equivalent previous proven lived experience.

oExperience of liaising with multi-disciplinary professionals and working across agencies oExperience of delivering training oExperience of working with families oExperience of achieving change and unblocking the system oExperience of service development and change management oExperience of supervising others Desirable Have experience and skills in undertaking holistic whole family assessments and risk assessments.
Knowledge Essential oA proven understanding of the needs and characteristics of children and young people with a diagnosis of Autism and or complex communication difficulties oStrong, holistic understanding of the education, health and care system to fulfil keyworking function of holding services to account oExperience of working with children and/or young people with learning disability, autism or both and complex needs and their families oA proven understanding of the difficulties likely to face children and young people with Autism and or complex communication difficulties oA proven understanding of strategies and resources that will support children and young people with Autism and or complex communication difficulties oA proven understanding of the role adults play in supporting children and young people with Autism and or complex communication difficulties oAn understanding of equal opportunity issues and an awareness of what this involves, being able to communicate with people from minority groups, people with disabilities oStrong understanding of children's mental health and human rights oAn understanding of the challenges experienced by Parents of Children and Young People (CYP) with Special Educational Needs and Disability (SEND) and Autism oLegislative knowledge and awareness of locally available provision, as well as how to apply in practice oAn understanding of safeguarding procedures Desirable Some experience of facilitating groups would be advantageous.
