Job summary This new Macmillan service aims to ensure that adults affected cancer experience excellent psychosocial care wherever they are on the cancer pathway.
This is an integrated whole system model for identifying and providing psychosocial support across the cancer landscape.
Active service user involvement is fundamental to the service aim to support personalised comprehensive care and support for people affected by cancer in our population through: Timely & equitable access to appropriate level 1-4 (NICE, 2004) mental health support integrated with cancer care clinical pathways Improved coordination, proactive and integrated psycho-social support across primary, community, social and acute settings Psychosocial education and support for health care professionals (including collaborative learning and reflection sessions) and ensuring continuous development and improvement of the psychological care approach Improved integrated support for people with learning disabilities and pre-existing mental health conditions with cancer This service has been funded collaboratively by Macmillan and Lancashire and South Cumbria Cancer Alliance with the support of Lancashire and South Cumbria ICB.
The post is based at Preston Business Centre, however, the post holder will be expected to travel to sites across Lancashire and Cumbria.
Main duties of the job Develop effective working relationships with the following stakeholders: o Macmillan Psychosocial Pathway Lead o Mangers who lead on related work streams o All levels of management within LSC ICB and LSC Cancer Alliance The post holder will be the Project Manager for the Macmillan Psychosocial Pathway for Lancashire and South Cumbria and will work autonomously, referring to the Pathway Lead and Macmillan Pathway project Lead when required.
The post holder will have overall responsibility for the integrity and coherence of the pathway.
There will be three main elements of this role: project management, gathering information/data as the evidence base for the Macmillan Psychosocial Pathway and stakeholder engagement.

Job description Job responsibilities Responsible for supporting the Macmillan Pathway project Lead to deliver the objectives of the pathway, working with a range of internal and external stakeholders both across Lancashire and South Cumbria.
Maintain all related project documents for the project in line with LSCFT and Macmillan requirements and organisational standards.
To help develop digital solutions which work across LTHTR, UHMB and third sector.
Responsible for coordinating the gathering, analysing, and reporting of information and the evidence base for cancer performance activity, pathways, services and patient experiences in Lancashire and South Cumbria and in commissioned providers and across the local authority to inform the development of the models of care.
Develop communications and support engagement plans for the Psychosocial Pathway Project and manage their effective implementation.
Travel is a requirement of this post.
